Job overview

An exciting opportunity is opening up for an experience senior clinician with either Professional AHP or Nursing Qualification to work with the Northwich Care Community as a Complex Case Practitioner. 

Working with complex patients with various long term conditions and/or multiple complex social needs you will provide holistic clinical assessment to optimise individual patient specific health needs with a real focus on patient identified goals. You will have experience in motivating and engaging patients, multidisciplinary working and supporting the development of the community team.  

Main duties of the job

The post holder will be responsible for proactively providing the support /short term case management/case co-ordination of people within the GP cluster who are at medium/high risk. The post holder will support this process by assessing and supporting the individual to design their care and to support independence at home or in the Community.

The post holder will be able to support a patient’s Health and Social Care needs, as well as have the ability, to holistically assess changes in the patient’s condition, within agreed pathways.

Develop close working relationships with local health, Social Care and Community organisations as part of the Integrated Care Agenda.

To work autonomously within the Care Community Team working within and across professional and organisational boundaries.

Detailed job description and main responsibilities

Be responsible for the holistic and clinical assessment, planning and implementation, evaluation and facilitation of care to individuals, groups of patients and their families on the caseload encompassing a proactive, rehabilitative and an active self-care approach. To act as an autonomous practitioner taking on going responsibility for the complex care coordination of appropriate patients on their caseload.

Contribute to the organisation of multi-disciplinary, patient centered care. Assess and develop an individualised plan of care to enable proactive management of patients with complex needs, which may be identified by a risk stratification process. Develop and support reduction in hospital admission targets within Integrated Care Strategy.

Take an active role in health education of the individual and community by promoting self-care for long and short-term conditions, utilising motivational interviewing techniques and provide both planned and opportunistic care based upon evidenced based information.
