Job overview
The Community Advanced Clinical Practitioner will contribute to the delivery of care within the Integrated community team using advanced clinical assessment skills to support timely assessment, intervention and management of the people presenting with acute, complex and / or long term conditions in a variety of settings across the community footprint / neighbourhood.
The position is working in a community setting, visiting patients in their own homes to support them to avoid unnecessary hospital admissions. You will drive the ACP agenda and support the clinical lead to develop the service for our patients.
The successful candidate will ensure they achieve all 4 pillars of advanced practice. Predominantly they will undertake clinical assessment of complex older people, aiming to reduce unnecessary hospital admissions. They will support he virtual ward and the urgent community response service as well as working with the Primary Care Network.

Main duties of the job
The Community ACP will: -
- Undertake assessment of patients with differential and undiagnosed illness or exacerbation of existing conditions using advanced clinical skills, critical thinking, decision making and risk analysis, to provide safe, high quality, evidence-based care preventing unnecessary acute hospital admissions, supporting patients in their own homes for longer and preventing premature admissions into long term care facilities
- Provide advanced clinical assessment of patients including appropriate diagnostics, early interventions to determine a clear clinical management plan and ensuring that the right care is received, in the right place at the right time.
- Take a lead clinical role within the community team supporting the triage and streaming function advanced clinical assessment and as part of the integrated team.

Qualifications
Essential criteria
- Hold a current professional registration such as NMC (Nursing and Midwifery Council) or HCPC (Health and Care Professional Council)
- First Level degree or relevant qualifications
- MSc (Master of Science) Advanced Practice or equivalent Clinical Masters (to include as a minimum clinical examination/diagnostics, independent Non-Medical Prescribing, Leadership, and research)
- Recognised teaching/ assessing qualification e.g. ENB 998: City and Guilds 730(7) Teaching and assessing adults
Knowledge
Essential criteria
- Demonstrates attributes of a clinical expert in terms of specialist theoretical and practical knowledge/ expertise in advanced clinical reasoning and the range of work procedures within Acute Frailty care in the acute and emergency care setting
- Advanced knowledge of the pathology of disease and illness processes relevant to community care including chronic disease, cancer, and end of life care and long term conditions
- Evidence of professional/clinical knowledge managing acutely unwell patients in the community care setting
- Advanced knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
- Clear and demonstrable evidence of advanced, autonomous practice in the management of patients / service users
- Knowledge and understanding of Clinical Governance and how it applies to role
- Knowledge of mental health assessment and safeguarding vulnerable adults
- Knowledge of infection control
