Home oxygen assessment nurse based in the community. To provide accurate assessment of patients oxygen requirements, oxygen prescription based on this assessment, appropriate follow up, risk assessments and education.
Prif ddyletswyddau'r swydd
To accept patient referrals and assess , plan ,implement and evaluate appropriate oxygen therapy related prescriptions and care plans.
To perform and action the results of ambulatory oxygen assessments .
To review patients at home who are receiving oxygen therapy and update oxygen prescription and care plan as clinically indicated.
To carry out community visits to patients on the Virtual Ward.

- Nursing qualification – Registered Nurse Degree level
- Clinical examination /assessment qualification

- Non medical prescriber (V300)
- Module in Education in Practise
Knowledge and Expertise

- An in-depth knowledge of evidence based health interventions
- Chronic Disease management in accordance with NICE guidelines and DOH
- Client confidentiality, including data protection and Caldicott guidelines

- Knowledge of Local Area
- Previous knowledge of Audit and use within clinical services.
Experience

- At least 3 years post registration experience
- Evidence of training/working in respiratory Care
- Experience of providing client/patient centred services in a healthcare setting

- Experience in Primary care
- Experience in use of computer data and record systems
