The post holder will contribute to the provision of specialist palliative and end of life rehabilitation to the people of Cheshire with malignant or non-malignant conditions who have a limited life expectancy, working autonomously and as part of a team. They will deliver therapeutic interventions in their home and/or when they attend the Hospice as an out-patient, in-patient or attend day therapy, providing advice, education and support to patients, their carers and other health care professionals. Over recent years there has been a lot development within the service and the successful candidate will need to share the teams drive to take the service forward. You also need to be passionate about delivering quality care to people receiving palliative or end of life care.
