Hywel Dda University Health Board are seeking a Band 4 Dietetic Assistant Practitioner to work as an integral member of the  multi- professional Weight Management Team.

This is an exciting opportunity to contribute to the implementation of the All Wales Weight Management Pathways (AWWMP) working in collaboration with key members of the MDT and strategic partners to support delivery of the Healthy Weight Heathy Wales strategy.

Main duties of the job

The successful applicant will work with the Weight Management Team, key partners and stakeholders to plan, deliver and evaluate weight management programmes and interventions at level 2 of the AWWMP,  provide dietary and lifestyle education and training to service users and key partners and autonomously support and manage a case load of service users.

You will demonstrate a good understanding of the role of nutrition in health care, be an excellent communicator, a team player and be prepared to undertake further learning and development to fulfil the scope and requirements of this role.   

You will join our t highly respected Health Board Weight Management team and benefit from a well-established friendly and supportive structure, an active focus on continuing development and well-established multi-professional links.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:

Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;

48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;

Qualifications and knowledge

Essential criteria
  • NVQ Level 4/Diploma in nutrition / health promotion
  • Good awareness of equality and diversity issues
  • Knowledge of the principles and practices involved in behaviour change
  • Demonstrate sound knowledge of the importance of good nutrition health
  • Understanding of the evidence base underpinning routine lifestyle/health improvement practice
  • Further evidence of continual education
  • A commitment to undertaking continuous professional development
Desirable criteria
  • Motivational interviewing
  • Agored Cymru, level 2 Community Food and Nutrition Skills
  • Group educational and training Skills
  • Accredited courses in behaviour change
  • Good knowledge of government initiatives about long term conditions and self-care
  • Knowledge of NHS in particular health improvement
  • Knowledge of voluntary organisations

Experience

Essential criteria
  • Experience of working in a healthcare/community setting
  • Maintaining routine administration systems
  • Experience of communicating with effectively with a range of people at different levels
  • Experience of involvement in development work e.g. gathering data for monitoring and evaluating to inform service improvement
  • Experience of delivering group education
Desirable criteria
  • NHS experience
  • Evidence of involvement in supporting nutrition/lifestyle improvement programmes
  • Experience of project work
  • Experiencing of delivering health brief intervention
