Job overview

NHS Wales has an ambition to provide care closer to home to patients who would otherwise be admitted into hospital. West Wales is at the forefront of managing those patients that are more acutely unwell or complex preventing avoidable admission or expediting earlier discharge through our clinical streaming hub, crisis response and hospital at home services.  This role plays a pivotal role in providing the day-to-day clinical oversight and supervision of delivery of these services.  

This is an exciting opportunity to be part of the development and implementation of the Health Boards Intermediate and Urgent Primary Care model.  You will be joining a growing cohort of similar clinicians and a multidisciplinary team who are passionate about providing holistic person-centred care and managing risk to support people in their own homes.  You will be working as part of the wider system with similarly minded clinicians including SDEC and Frailty, OOH, WAST, and Primary care working together to ensure a joined up integrated coordinated response with a Home First ethos.  

Main duties of the job

We are looking for an innovative, proactive individual who will join and enhance our integrated working model.

You will support the development and delivery of our enhanced community/Hospital at Home model, engaging with a wide range of clinical stakeholders for the management of patients on the virtual ward.  You will provide clinical leadership to the wider multidisciplinary team and work at the interface between acute care, acute frailty, Out of Hours and existing primary and community services.  You will deliver high quality specialist assessment/interventions and use appropriate point of care testing, digital monitoring and support to facilitate care.  You will provide support of the development of pathways that support admission avoidance and / or early discharge particularly for frail older patients and shape the role of the urgent and intermediate care MDT and response.  You will provide mentorship and support to advanced practitioners/other urgent care clinicians.  You will clinical triage with Welsh Ambulance Services NHS Trust (WAST) and mitigating conveyance to acute hospital where appropriate and manage patients in step up/step down beds to support recovery, rehabilitation and timely discharge as part of the intermediate care response.

· Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Person specification

Qualifications and Knowledge

Essential criteria
  • Full GMC Registration with a license to practice and entry on the GP Register
  • Certificate of Completion of Training (CCT) in General Practice / Certificate Confirming Eligibility for General Practice Registration (CEGPR), or equivalent
  • Included on a Medical Performers List or eligible
  • Evidence of continuous professional development
Desirable criteria
  • MRCGP

Experience

Essential criteria
  • Broad based experience in Primary Care
  • Knowledge of UK Hospital systems (or equivalent)
  • Knowledge & participation in CPD
Desirable criteria
  • Experience of NHS General Practice, including Out of Hours
