Look no further! We're currently seeking two, highly skilled and driven Physiotherapists to join our Community Rehabilitation Team in Carmarthenshire.

As a Specialist Community Physiotherapist, you'll contribute to a paradigm that prioritises individual uniqueness and empowers patients to achieve optimal function and vitality. The Physiotherapy Community Team is rapidly evolving. This team is dedicated to the Health Board's commitment to building community capacity and improving access to rehabilitation as close to home as possible.

The post holder will work closely as part of a broader, larger, dynamic multi disciplinarily teams (MDT) of Occupational therapists (OT’s), Nurses, Social Workers, Dieticians, Community Pharmacists and Third Sector Services to name a few.  In order to provide timely, patient centred, seamless care integration with the Acute Hospitals, Community Hospitals, Front of House Services (TOCALS) and Intermediate Care Services is essential.

Main duties of the job

The post holder will assess, develop and implement co-produced, personalised treatment plans for patients with a diverse range of conditions in their one home, community-based classes (including aquatic therapy) and outpatient clinics. We place a strong emphasis on patient-centred care to foster independence and well-being. 

As a versatile postgraduate, you will need to have a strong clinical grounding in older people rehabilitation, complex manual handling, and knowledge in diverse conditions.

The post holder will contribute to spearheading initiatives to implement progressive rehabilitation strategies, service development and contribute to our organisation's reputation for innovation.

Successful applicants will require an excellent ability to work autonomously, managing and prioritising a diverse caseload, have proven clinical reasoning/problem solving skills and provide comprehensive and professional interventions.

The post holder will possess motivational and developmental skills to support and supervise junior Physiotherapy staff, therapy support workers and students.

The post holder will be required to participate in the provision of a flexible 7-day service, based on service need and rota basis. They will have the skills required to participate in the respiratory On-call service.

Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Experience

  • Practical experience to a level that would enable participation on the respiratory on call rota
  • Therapeutic handling and risk assessments
  • Managing a diverse caseload using a clinical reasoning process
  • Inter-professional and team working
  • Direct involvement with carers and families
  • Evidence of supporting service compliance with clinical governance requirements
  • Experience of Staff supervision
