THIS POST IS FIXED TERM/SECONDMENT FOR 12 MONTHS (31.07.25) DUE TO THE NEEDS OF THE SERVICE 

If you are a Hywel Dda University Health Board employee and are applying for this post as a secondment, agreement for the secondment is needed from your current manager prior to application.

This post comes during a period of Service Transformation in response to the changing needs of people with a learning disability and the national agenda for improvement. If successful, you will play a vital part within this exciting period of change, by contributing to the design of a new service model providing flexible & responsive services.    

You will have the opportunity to work closely with other colleagues across the LD service and the Third Sector, providing specialist support, advice and training to patients, families, carers and professionals in Primary Care or within the hospitals.

Applicants should have experience of working with people with a learning disability.  Additional training will be available, and you will be supported to develop your own clinical competency profile & to develop skills within Learning Disabilities.

Main duties of the job

We are looking for a suitably skilled and motivated healthcare support worker to join our Learning Disability Liaison Team covering the three counties. The Liaison team provides support to people with a learning disability to improve access to Primary Care services and those admitted to General Hospitals.

The main responsibility for this role is to reduce health inequalities for an individual with a Learning Disability. You will be required to work within General Hospital settings and Primary Care settings i.e GP Surgeries to ensure that individuals receive the highest standard of care from a Learning Disability perspective.  

There will be an element of delivering training to staff, clients, families and carers so presentation skills are desirable for this post, however, training will be provided. 

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;

48 general practices (six of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;

Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
