Trosolwg o'r swydd

Could you make a difference to someone who has had a life changing spinal or acquired brain injury event, through physical activity, meaningful activity or helping to communicate their needs?

We are seeking new members to our team who share our passion for the care and holistic rehabilitation of patients that have experienced life changing injuries.

The Band 4 rehabilitation coach roles will work across both our specialist spinal and neuro rehabilitation wards in the spacious and modern new rehabilitation centre at University Hospital Llandough.

Prif ddyletswyddau'r swydd

The innovative Band 4 Rehabilitation Coaches will support the practice of the multidisciplinary team (including Dietetics, Nursing, Occupational Therapy, Physiotherapy, Speech and Language Therapy and Clinical Psychology) to deliver the therapeutic goals of people undertaking rehabilitation.  They will also be responsible for delivering person centred meaningful activities working closely with patients, their relatives and carers, volunteers as well as the professionals involved.

A key aim of the role is to deliver rehabilitation across 7 days to improve flow and enhance the patient experience.  Therefore, rotas will include weekends and bank holidays as well as week day shifts.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications

  • Good standard of education, GCSE English and Maths (Grade A-C) or Level II equivalent qualification.
  • NVQ/QCF Level 4 or equivalent level of knowledge Or NVQ/QCF Level 3 PLUS Additional training to Diploma level or equivalent or Equivalent through short courses and experience and equivalent competencies achieved and evidenced through accredited courses.

  • Health and Social care related qualification

Experience

  • Relevant experience of working in a rehabilitation environment
  • Organisation of delegated workload to support direct patient care.
  • Inter-professional team working.
  • Involvement with carers and families.

  • Experience of working across statutory and voluntary agencies
  • Previous therapy assistant experience
  • Working with people with neurological conditions including behavioural and communication disorders

Skills

  • Acts in accordance with Code of Conduct for Healthcare Support Workers in Wales
  • Basic IT skills.
  • Able to use analytical skills and judgement.
  • Excellent communication abilities and team work skills.
  • Demonstrable skills in the facilitation and co facilitation of therapeutic group work.

  • Knowledge of spreadsheets and databases.
  • Preparation and planning of own groups in line with the Rehabilitation plan.

Special Knowledge

  • Knowledge of Health & Safety at Work and related issues.
  • Knowledge of teaching skills appropriate to adults, and knowledge of learning styles.
  • Knowledge of the presentations associated with physical and cognitive difficulties.
  • Knowledge of the range of Rehabilitation interventions appropriate in the treatment of people with physical and cognitive difficulties.
  • Knowledge of the roles and responsibilities of the other professions in the multi-disciplinary and multi-agency team.

  • Knowledge of the application of practice in a range of therapy professions.
  • Awareness of the principles of the implementation of the Unified Assessment Process.
  • Awareness of the principles of client centred goal setting.
  • Knowledge of the procedures pertaining to the ordering, monitoring and control of stock equipment.
  • Demonstrates awareness of disability/impairments
