This is a 10 PA substantive post in community adult psychiatry based in Peterborough with the Peterborough Adult Locality team (PALT) and the primary care mental health service (PCMH). There is a generous recruitment premium attached to this post of £40K in addition to the standard relocation expenses.

The post is based in Peterborough City centre (Town hall) with opportunities to work remotely during the week. You will work with 4  other PALT consultant psychiatrists , each  with responsibility for a geographical patch covering approx. 6- 8 GP practices. You will be working closely with a small team of senior clinicians who will manage the flow of patients from  practices between primary and secondary care. You and your team will provide assessment and treatment for all patients in your area.  You will work within the Locality team, which is itself sectorised, to manage the most severely unwell patients in your sector but you will also be aligned to staff from the Primary Care Mental Health Service who will support and assess patients in primary care from your key practices and step patients up into the team when needed. These PCMH teams are joined, as a result of the bid, by social workers, a pharmacist and physical health care workers and are further enhanced by a therapy group program for patients not held in secondary care.

To provide clinical expertise and leadership to Peterborough Adult Community Mental Health services, integrating mental health care across primary, secondary and 3rd sector care systems.

The post holder will be supported by 2 existing senior substantive consultants within the team. There is an excellent network of wider consultant support in Peterborough with weekly academic meetings and the opportunity to join a well-established consultant peer group for adult consultants in the North.

The locality team has a WTE staff grade Dr post. The team regularly hosts core Trainees, and higher trainees. The post holder would be encouraged to be both a clinical and educational supervisor for trainees.

Key Clinical Responsibilities

The post holder will provide clinical leadership to the team. He or she will contribute to assessment of patients referred to the service for diagnosis and treatment of mental disorders and support MDT members who carry out assessment of patients both within the team and in primary care. The post holder will help to facilitate accurate diagnosis and timely interventions so that treatment can be delivered in a timely manner and  in accordance with the recovery model. He or she will also provide ongoing treatments to patients that are evidence based and supported by NICE guidelines maintaining a bio psychosocial approach.

The post holder is required to maintain Section 12(2) status and Approved Clinician status and have a comprehensive awareness of Mental Health, Mental Capacity and Care Act legislation. He or she will be expected to participate in Mental Health Act assessments when possible and will be the Responsible Clinician for patients on supervised community treatment orders in the locality.

The post holder is expected to be committed to a multidisciplinary approach to care and actively contribute to MDT meetings and formulation meetings and work in close collaboration with team members, care coordinators, other CPFT services, third sector and statutory agencies as required for the care of community patients. The post holder is expected to develop close working relationship with primary care, particularly General Practitioners and support primary care clinicians to manage patients with mental health difficulties within the primary care setting where appropriate. Consultants will be expected to form close working relationships with their allocated group of GPs (including regular visits to GP surgeries) and to provide support and supervision to the Primary care Mental Health team in their area, as well as to provide care to patients needing specialist mental health care.

There are opportunities for both clinical work (including consultations  and advice and guidance) and supporting professional activities to be conducted remotely through the use of video consultation software and videoconferencing software.

The post holder will be expected to contribute to the collection of clinical information to ensure further service development and to enable audit, participate in clinical governance activities and contribute to the day-to-day management and to the overall service development agenda through collaboration with managerial and clinical colleagues.

Special interest opportunities:

The post comes with excellent opportunities and support from the Directorate to develop a   special interest in line with service needs. A proportionate reduction in Clinical PAs (from 7.0 to 6) may be an option dependent on candidate’s individual interests if successful at interview.

Example opportunities include:

  • Medical education: CPFT has a strong medical education department and consultants with key formal roles in the North. Opportunities exist to develop as an educator at undergraduate and post graduate level. Peterborough is part of the Cambridge university ‘northern hub’ and hosts clinical students in psychiatry throughout their 3 clinical years. The Northern Regional Clinical Subdean for Cambridge University medical students is based at Peterborough City Hospital and the CPFT Northern Undergraduate tutor at the Cavell Centre Peterborough. There is an excellent mentoring scheme for medical students interested in a career in psychiatry, PsychStart Cambridge, run by the Northern Undergraduate tutor in conjunction with RCPysch. Links with the tutor and Subdean will easily facilitate a more formal role with undergraduates at Cambridge University e.g. in examining, clinical skills facilitation, and professionalism facilitation. CPFT’S   Foundation Year Tutor and RCPSsych Foundation Fellowship Mentor, is also based in the North of the Trust. There is also a postgraduate tutor role based in the North for our psychiatry and GPVTS trainees. You will be encouraged to train as an educational and clinical supervisor for foundation, GPVTS and core/higher trainees in psychiatry. Interest in developing a specific role within medical education can be discussed with CPFT director of Medical Education Dr Thirza Pieters as to how this might combine with the post.  .

Candidates with an interest in medical education would be encouraged and supported to enrol on the Cambridge University Integrated Foundations of medical education (IFME) programme/Post graduate certificate/Diploma in medical education depending on their training needs

  • Medical management and leadership. The NHSE exemplar project provides the perfect opportunity for someone interested in furthering their skills in service development, medical management and leadership. The evaluation of the outcome data from the project will shape the future of mental health services nationally. It is expected that the evaluation will be presented at national and international forums and published in high impact psychiatric and health service journals. There are many aspects of the NHSE project that would make an ideal dissertation for someone undertaking an MA in medical leadership/management. A candidate interested in medical management and leadership would be encouraged and supported to enrol with appropriate training though local and wider NHS leadership programmes according to their level of existing training. Dr Catherine Maxey Clinical Director and Dr Julie Hankin Medical Director would be happy to discuss further specific roles within medical management for interested candidates and how they might combine with the post.
  • Areas of clinical special interest: candidates who have a specific clinical area of interest (e.g. Substance misuse, transcultural psychiatry, affective disorders, psychosis, physical and mental health overlap) that they wish to develop would be encouraged to do so and become the clinical expert across the locality and wider. Candidates interested in a particular clinical subspecialty are encouraged to discuss with Dr Maxey as to how this might fit with the post.
  • Research: The Peterborough Exemplar evaluation is affiliated to the NIHR Applied Research Collaboration in the East of England and is led by Professor Jesus Perez. There is a dedicated research associate in residence and there are opportunities to join and or lead this evaluation. CPFT has an active research portfolio as a Cambridge university partner Trust as detailed in Appendix 1. Candidates with a specific interest in furthering research through this route are encouraged to discuss with Professor Perez (Consultant Psychiatrist and Clinical Director of LCRN) or Dr Maxey as to how their interests might fit with the post.
