Job overview

To work in conjunction with the Multidisciplinary Team and in line with Local and National Targets to provide specialist evidence based nursing  advise to support and facilitate high quality, individualised care to  patients/clients and relatives/ carers without supervision

This will involve dealing with complex situations and having to take appropriate  action in a variety of settings. 

Provide expert advice and education for Healthcare professionals  involved in the care of people with diabetes and their carers. 

Acting as a resource for general practice and working to reduce  admissions, improve diabetes outcomes and quality of life for people with diabetes. 

To provide advanced clinical practice in the field of diabetes care and ensure the provision of a quality patient focused diabetes service in Cambridgeshire and Peterborough

DSN to be based at the Healthy Living Centre in Peterborough and preferably have experience in managing patients with type 1 diabetes including Young people and those who are using insulin pumps

We will need T1 / Pump and possibly also Young Persons experience.

 

Main duties of the job

Work collaboratively with the team and professional leads to support professional and practice developments including CPFT clinical supervision framework.

Work autonomously to assess, plan, implement and evaluate programmes of care, providing specialist advice and actively seeking evidence based practice which improves the quality of care and meets the needs of patients. A holistic approach to caring should be planned that will include the appropriate level of physiological; pathological; spiritual, socio-economical and cultural aspects which influence the care of clients within your specific speciality.

  • To manage a significant caseload of patients with complex diabetes, managing individual programmes of care working with patients for self management of their diabetes.
  • Provide community nurse led clinics within general practice.
  • Work within the MDT diabetes team supporting general practice within virtual review clinics.
  • Work proactively to prevent admission to secondary care
  • Run clinics for patients on insulin pumps in conjunction with the consultant
  • To provide education for patients with type 1 diabetes and type 2 diabetes

Detailed job description and main responsibilities

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.   • To promote and advance non medical nurse prescribing for the benefit of people with diabetes.
  • Develop care plans and treatment protocols for patients with diabetes in primary care.
  • Involvement in clinical audit, present and evaluate said audits.
  • Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.
  • The post holder must adhere to the Trust Risk Assessment and Risk Management Processes
  • The post holder must adhere to Infection Control Policies and Procedures.
  • It is a condition of your employment that you are currently registered with the NMC and it is your responsibility to maintain your professional registration.
  • Participate in clinical supervision on a regular basis and  be responsible for the management of and reporting on individual and team performance indicators and development of remedial action plans as required, The post holder must also participate in clinical and other audits as required.

Person specification

Education / Qualifications

Essential criteria
  • First level registration ENB 928 or equivalent in diabetes nursing
  • ENB 998 or teaching certificate or experience in training and education
  • Evidence of degree level learning
  • Evidence of continuous professional development
Desirable criteria
  • Degree in health related subject
  • Non medical prescriber Diabetes educator in DESMOND / PDAC or Equivalent
  • Counselling qualification

Experience

Essential criteria
  • Experience in managing complex diabetes
  • Experience in providing specialist education to patients, nurses and other health care professionals Ability to run own caseload in different settings.
Desirable criteria
  • Experience in audit/research/project management
  • Experience of working in a community setting

Skills & Abilities

Essential criteria
  • Evidence of expert clinical skills including understanding of insulin pump therapy, structured education programmes and newer diabetes therapies
  • Standard/advanced keyboard skills and ability to communicate through IT using packages such as Word, Outlook, Excel and PowerPoint . (it is best to be specific i.e. is it for data entry, analysis, formulas, creating a data base)
  • Report writing skills
  • Ability to work flexibly as part of a team.

  • Ability to use SystmOne
