Job overview

An exciting opportunity has arisen for an enthusiastic  Children's Community Palliative Nurse Specialist  to join the  Children’s Nursing Team within Cambridgeshire Community Services NHS Trust – Luton and Bedfordshire Locality.

Our innovative team aim to provide skilled nursing care to children with a range of medical conditions, both acute and chronic at home and other community settings.   

We strive to prevent hospital admission and promote early discharge for children of Luton and Bedfordshire.

This role  requires the post holder to  provide direct clinical care  for children with palliative conditions including participation in an on call service to provide care for children at end of life. The post holder will be an expert resource for children’s palliative care and provide education and training to both the Luton and Bedford  Children’s Community Nursing (CCN) Teams and the wider multi-disciplinary team.

Main duties of the job

The Children’s Community Palliative Nurse Specialist will work within the Children’s Community Nursing Team in the community settings across Luton and Bedfordshire.

To work as an autonomous practitioner, using expert assessment and symptom control skills and knowledge to maximise a child or young person’s quality of life and support their families and carers. 

To be part of a professional team, including Children’s Community Nurses, Community Nursery Nurses and the wider team; the Hospice, GPs, Acute and Community Paediatricians and Social Care to ensure the appropriate support is available to all palliative children and young people with palliative.

There may also be occasions where it may be necessary to support the CCN with the clinical caseload.

Detailed job description and main responsibilities

To be an expert resource for Children’s Palliative Care skills and knowledge. and provide education and training to the CCN and Specialist Nursing Team and the wider multi-disciplinary team.

 You may be required to participate in the on-call rota for the service.

To be the lead Clinician within the CCNSN Team for the specialised area of children’s palliative care; this may include networking with specialist nurses in other areas to develop practice where appropriate and sharing knowledge and skills with colleagues.

To work in conjunction with the named professionals for individual children/young people with palliative conditions, acting as a resource to improve healthcare outcomes (for example utilising a step up / step down approach).

To lead, develop and establish integrated clinical care pathways across CCNSN services to ensure a consistent approach to case management/care coordination.

To work with the Children’s Services Team Managers to ensure that the resources and skills of the team are used effectively and appropriately to meet the needs of the children/young people and their families, thereby helping to identify any gaps in service provision.

To liaise and work collaboratively with professionals in primary, secondary and tertiary healthcare settings to ensure timely discharge planning and symptom management using an integrated palliative care pathway following national guidelines.

To co-ordinate/facilitate the implementation of complex symptom control regimes plans and lead specialist palliative care community nursing interventions utilising a range of clinical skills. 

You will be expected to practice as a prescriber within an agreed scope of practice as part of this role.

Treat symptoms through pharmacological and non-pharmacological strategies. Undertake diagnostic testing within competence, e.g. Phlebotomy.

To empower health and social care professionals to deliver quality evidenced based care service by sharing knowledge and expertise.

To support the implementation of transition pathways from Children to Adult Services for children with palliative needs, ensuring a seamless pathway

 

Person specification

Qualifications and Training

Essential criteria
  • RSCN/ Registered Nurse (RN) Child Branch - with evidence of post qualifying education (to a PG dip level (Level 7) or evidence of working at that level.
  • RN Adults, RN Learning disabilities or Registered Midwife with SCPHN, SPQ or ACP qualification, WITH extensive evidence of Managing children with Palliative care post graduate training in the community or willingness to complete within two years.

Experience

Essential criteria
  • Working in the palliative care field with babies, children, young people, with sound knowledge of symptom management and assessment of their health needs.
  • Evidence of teaching/training experience.
  • Audit /evaluation of service/project implementing / reviewing clinical care delivery changes in practice
  • Ability to evidence knowledge on the wider palliative care agenda
Desirable criteria
  • Experience of caring for children and young people with an oncology diagnosis
  • Experience of caring for children and young people with long term conditions Experience of working with families and carers to promote health and wellbeing of their CYP
