Job overview
The Children’s Community Specialist Nursing Service, based in Cambridgeshire, is looking for a Children’s Complex Care Sister/Charge Nurse to join our growing team.
The Children’s Complex Care team work in a collaborative, enabling and inclusive manner with all. With the aim to advocate for individuals and their families to deliver a bespoke complex high-quality service to children and young people within their community and educational settings. We will achieve this by honouring our values of honesty, empathy, ambition, and respect of children, young people, and their families with complex physical health needs.
We also support families with their care co-ordination and training by offering expertise and advice appropriate to their needs.

Main duties of the job
Complex nursing care co-ordination for children and young people with complex physical and health needs.
Joint working across the Children’s Specialist Community Nursing Service to provide training and complex case management within the counties Special Needs School and provide skilled nursing care to children and young people in their own homes.
Acting as an autonomous practitioner, which includes being Nurse in charge - allocating and delegating daily to registrants and non-registrants.
Responsible for daily management of the team and escalating where necessary.
Provide clinical leadership and direction for the Complex care team.
Undertake nursing assessments of children and young people to inform the Children’s Continuing Care framework.
To provide high quality, evidence-based community nursing care within a defined locality, promoting a high standard of nursing care for children and their families.
Support the Band 4 Health Care Support workers and external stakeholders in delivering evidenced based care or within the child’s familiar environment (e.g. their home, school or community).
Work within the established Children’s Continuing Care Team providing evidence based short break care for children who meet NHS continuing care criteria within the home.
Supporting children and young people who have palliative and end of life requirements, alongside providing support for their families.

Detailed job description and main responsibilities
Maintain the safety of children and young people by ensuring a robust complex needs health care plan are in place.
Devising care packages that have been commissioned by the Integrated Care System (ICS).
To assess, plan, implement and evaluate the nursing care and support requirements for children with complex health needs.
To engage with children and their parents/carers along with other agencies who are involved in their care to promote a person-centred outcomes approach to supporting their needs.
To provide support to training and competency assessments and ongoing clinical supervision for Health Care Support workers/Administrative Clinical Assistants within the CCNT.
To provide theoretical training and competency-based assessments in relation to nursing interventions for staff working in partner agencies including schools (mainstream and special needs), colleges and early years settings.
Ongoing reviewing, critiquing and improvement of the training that the service provides both internally and externally.
Undertake Nurse in Charge responsibilities to triage day-to day communications and referrals alongside other Shift Leads within the service (Acute CCN and Specialist Nursing for Schools Service).
To support the Lead Nurse in developing service improvements to ensure the health needs of the children and young people who use the service are met.
Actively engaging in the development of processes and procedures within the complex care team.
If you have a non-medical prescribing qualification, you will be expected to practice as a prescriber as part of this role.

- Experience of working with children and young people with complex health needs.
- Experience of working with families experiencing complex and emotive situations.
- Can demonstrate how s/he has promoted ‘best practice’.
Desirable criteria
- Experience of working with patients who require tracheostomy/long term ventilation.
- Experience of working within palliative and end of life care
Skills
Essential criteria
- Excellent communicator with attributes of active listening, compassionate and clear verbal and written communication.
- Demonstrates understanding of safeguarding issues.
