Diabetes Care Technician

A fantastic opportunity has arisen for a Diabetes Care Technician to join our Cambridge Team.   If you have compassion, empathy with the ability to work automatously this is the opportunity for you. 

You will be part of a multidisciplinary team and work under the supervision and guidance of The Diabetes Specialist Nurses (DSN). Your primary responsibilities will include supporting insulin treatment, gathering and providing feedback on Blood Glucose Levels, training patients on medication, insulins, and equipment, downloading patient BGLs and information, setting up CGMs for DSNs to review, participating in DESMOND/Patient T2DM Education, and collecting initial information calls to provide comprehensive information to DSNs for informed triage.  DCT for the Cambridge Team will be require to carry out home visits at the request of the DSN.  Peterborough DCT will be expected to support the Consultant clinics running from the Healthy Living Centre.

The role includes promoting a better understanding of Diabetes for patients, their families and carers, and other agencies involved with the patient through education, support and counselling.

To work as part of a team, under the supervision and guidance of the Diabetes Specialist Nurses, in the support and care of people with diabetes, in a primary care, community and domiciliary setting.

To work to empower people with diabetes to manage their own condition more
effectively and to ensure that the service meets the needs of all client groups.

 To understand and support the needs of carers and liaise with other relevant agencies. To work with members of the multidisciplinary team as and when appropriate. To manage own caseload of patients who require ongoing management and support with complex diabetes needs.

  • Work within own sphere of responsibilities and competencies.
    Undertake agreed clinical activities with patients under the guidance of the Diabetes Specialist Nurse.
  • To prioritise and maintain own patient caseload and carry out delegated health care tasks.
  • To undertake assessments on patients with Type 1 and 2 diabetes.
    Initiate treatment, working with agreed protocols and review as required.
  • Record all findings in the clinical records and patient notes according to Trust policy.
  • Report and document any changes and variations in the patient’s physiological , psychological or social status.
  • To provide education and demonstrate the use of insulin pen devices, blood glucose monitoring equipment and subcutaneous injections.
  • Deliver training/education to the MDT, students and other agencies.
  • Evaluate learning and revise training according to feedback.
  • Empower patients to manage their diabetes more effectively by delivering education, health promotion and the demonstration of any relevant equipment.
  • Be a resource for service users to access and use community services and facilities.
  • Set up and facilitate blood glucose technique/monitoring clinics with patients, including lifestyle and diet advice for patients and other health care workers.
  • Resource and display relevant health promotion materials/leaflets.
    Book rooms, arrange invitations, and arrange speakers for group training sessions.
  • Contribute to the planning, monitoring and review of service delivery by attending team briefings and any other relevant meetings.
  • Be responsible for carrying out all administrative/clerical tasks required to ensure accurate and risk managed records.
  • Take and record messages clearly, to action if appropriate or pass on to the relevant professional.
  • Provide statistical returns, records and reports as requested and extract data for audit purposes as required.
  • Maintain stock of equipment and materials and responsible for maintaining quality control and calibration of medical instruments and/or equipment used by this service.
  • Participate and contribute to team meetings, case conferences and any other meetings relevant to the post.
