An exciting opportunity to work with the Parkinson's team at Buckinghamshire Healthcare NHS Trust as a Parkinson's Support Nurse.

As a Parkinson's Support Nurse you will be part of our service expansion programme, focusing on providing education to ward staff to facilitate comprehensive care and support to in-patients with Parkinson's syndrome during their hospitalization and discharge.

You will work collaboratively with the teams to identify people with Parkinson’s as they are admitted to the Emergency Department and provide ongoing support to the patients', their family and staff throughout the patient admission. As part of the in-reach role the successful candidate will support ward staff ensuring medications are given on time and via an appropriate route.

Your role will involve coordinating and facilitating a smooth transition between inpatient and community settings, ensuring continuity of care with individualised care plans on discharge.

By ensuring a discharge care plan is in place prior to discharge the patients and care givers in the community will have clear guidance about switching from patches to oral meds where appropriate, referrals back into the community services and this will be supported by earlier follow up (within 2 weeks) via telephone. You will be working in a supportive work environment with the opportunity to gain clinic and community experience of caring for people living with Parkinson’s Syndrome

The post holder will work closely and supportively with the Clinical Nurse Specialists and directly with the Department teams. This will improve nursing practice; improve staff morale and patient experience.

The Main Responsibilities will include:

  • Collaborate with interdisciplinary teams to assess, plan, and implement individualized care plans for patients with Parkinson's.
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs to help facilitate tailored discharge plans.
  • Provide education, guidance, and support to Hospital staff, patients, families, and caregivers regarding Parkinson's management, medications, lifestyle modifications, and community resources.
  • Coordinate with community resources, rehabilitation services, and home healthcare agencies to facilitate seamless transitions for patients post-discharge.
  •  Coordinate with ward staff and the MDT to facilitate appropriate care and therapy for Patients in hospital.
  • Hold hot clinics to review deteriorating patients to reduce hospital admissions
  • Advocate for patients' needs, ensuring access to appropriate healthcare services and resources.
  • Document patient care, assessments, and interventions accurately and in a timely manner.
  • Follow up in patients homes post discharge to ensure effective discharge and ongoing care and support.
