Job overview

Cardiac Rehabilitation Specialist Nurse, caring and supporting cardiac patients who have undergone cardiac interventions/surgery, by contributing to a Rehabilitation Programme, where health education, promotion and modifiable risk factors are taught and provided.

The Post Holder will, as part of the Multi-Disciplinary Team, assist in the safe and prescribed delivery of care of cardiac prevention and rehabilitation service for Buckinghamshire NHS Trust. They will carry out cardiac risk assessments, making appropriate and timely referrals to associated agencies and work with the patient to provide risk factor modification with the aim of reducing the risk of further coronary disease. They will support the exercise specialists in the safe monitoring and delivery of structured exercise programs and therefore may be required to cover classes across site.

Assist in the running of the cardiac rehabilitation service and promote continuous development in clinical practice and delivery of patient care, encouraging innovation and appropriate change in service delivery within the speciality of cardiac rehabilitation in response to patient need, audit and evaluation and national guidance.

Main duties of the job

  • To assess patients in the clinic setting and provide them with rehabilitation and health education advice. Assess their suitability for commencing the cardiac rehabilitation programme.
  • To provide support, guidance and advice to patients and families with heart disease. To explain the implications of impending intervention and give advice about risk factor management.
  • To contribute to the case management of myocardial infarction and acute coronary syndrome patients and to liaise with other members of the health care team over patient care.
  • To facilitate input of data on both Health Machine and the NACR audit.
  • To assess with the Cardiac Rehabilitation exercise specialist, suitability and medical stability of all patient's exercise tolerance and advise on the rehabilitation process and exercise capacity and risk stratify individuals accordingly
