Job overview

An exciting opportunity has arisen for highly motivated individuals with at least 2 years recent renal/acute experience to join the  Chronic Kidney Disease Team.

As a Renal Specialist Nurse working within a nursing team you will be responsible for caring for patients with progressive chronic kidney disease, preparing them with individualised care and education for renal replacement therapies, transplantation or conservative management.

The department is open Monday to Friday 8am to 5pm. Closed weekends and bank holidays.

The advertised role is full time, there is some flexibility with working hours, although this will have to be within the departmental operating hours. Part time working may be considered if service needs are met.

Main duties of the job

In the CKD department you will assist in the management of patients with all aspects of chronic kidney disease, you will be involved in coordinating care for patients with CKD 4 to 5, preparing  and educating them for dialysis, or conservative management. You will be involved in anaemia management for patients with CKD. You will also participate in clinics off site at Walsall and Cannock, either nurse-led or as part of the MDT. You will also be expected to do home visits to patients as required. You will be working closely with the Transplant Team and assist in transplant education and workup.

 You will need to be a competent/confident individual as you will be required to work autonomously and will manage your own caseload of patients.

You will work closely with members of the multidisciplinary team and therefore must be an excellent communicator and possess excellent organisational skills.

You will be expected also to dictate your own letters from clinic reviews and make referrals to other departments and services when necessary.

You will have evidence of relevant post registration education. The  V300 prescribing prescribing qualification is desirable. Otherwise evidence that you are working towards it.

Co-ordinating care and facilitating care and reviews for patients with CKD 5, preparing  and educating them for renal replacement therapy (RRT), and also for patients being managed conservatively. 

You will be involved with anaemia management for patients with CKD 4 -5 for which cannulation skills  are desirable to be able to administer IV iron. Otherwise you will be expected to undertake cannulation skills training and competencies.

You will need to be a competent/confident and organised individual, as you will be required to work autonomously and will manage your own caseload of patients. You will be required to work autonomously, as you will be involved in clinics off site, at both Walsall and Cannock Hospitals, which can be either nurse-led or as part of the MDT. 

You will be expected to follow up any tests from your clinics and make changes where needed, liaising with the nephrologists and or senior specialist nursing colleagues. You will be expected  to dictate your own letters from clinic reviews and may have to make referrals to other departments and services.

You will also be expected to do home visits to patients under your care as required.

You will be working closely with the Transplant Team and assist in transplant education and workup. 

You will work closely with members of the multidisciplinary team and therefore must be an excellent communicator and possess excellent organisational skills. You will work closely with the dialysis and home therapies team when coordinating patients to start RRT.

You will have evidence of relevant post registration education. As a renal specialist nurse the  V300 prescribing qualification is desirable, otherwise you will be required to undertake this once established in post.

As part of the role induction you will spend time with the transplant team and home therapies team and haemodialysis team to be able to effectively educate patients about RRT. You will be expected to complete competencies within 12 months.

A UK driving licence  is preferable as part of the role involves working off site and visiting patients at home.

Qualifications

Essential criteria
  • Relevant post graduate specialty qualification or equivalent renal/ acute experience and knowledge
  • Current NMC qualification
  • 2 years post registration experience in acute setting
Desirable criteria
  • Health assessment module level 6 or above and non medical prescribing qualification

Skills

Essential criteria
  • Ability to apply knowledge and skills to a broad range of clinically and professionally challenging situations
Desirable criteria
  • IV cannulation competency achieved
