Job overview

This is an opportunity to join a Primary Care Network working for the Royal Wolverhampton Trust, made up of 9 GP Practices. The role will be helping to support the Practices with their data management, searches, targets, summarising, coding and searches. This has come at a time of great innovative changes in IT and technology in Primary Care and will be an exciting opportunity for someone experience in EMIS web and with a clear understanding of what is required for Primary Care

Main duties of the job

The post holder will centrally support practices within the RWT PCN in summarising and accurately coding relevant information from patient’s clinical records into the Practice computer systems efficiently.

Monitor and maintain patient call and recall systems to support RWT PCN practices to provide optimal patient care whilst facilitating a high standard of information entry to support both local and national enhanced service requirements (including QoF) and maximising income to the service.

This will involve promoting data quality to support the maintenance and development of clinical and information governance including the extraction, analysis, facilitation of audits and reviews to best practice standards.

Support monitoring of PCN information and performance requirements to enable the directorate to deliver improved patient care and maximize income opportunities.

The post holder will need to be able to build effective working relationships.

Detailed job description and main responsibilities

Reviewing medical records and producing an accurate summary of the patient’s medical history.

Ensure clinical correspondence is processed in a timely, safe and efficient manner whilst working closely with the GP Lead for workflow.

Accurately coding information into the practice’s clinical system within the PCN and raising any issues or improvements to the relevant members of the RWT PCN team.

Monitoring and maintaining patient call and recall systems.

Monitoring progress of notes summarising against Practice targets.

Using medical knowledge to correct mistakes found in existing patients notes and improve data quality across the PCN.

Assisting practices with queries relating to medical records.

Assist with the management of disease registers by supporting practices within the PCN with QOF processes ensuring data is captured and to a consistent standard.

Support practices within the PCN to achieve a high standard of information entry to maximise income against Enhanced Services and improve patient care.

Maintain clinical system audits and reports for all RWT PCN practices including auditing data collection standards.

Support development of EMIS Templates and protocols for use across the PCN.

Maintain comprehensive knowledge of all aspects of clinical systems, referring to training and support resources

Thorough experience of Primary Care essential

Essential criteria
  • Primary Care experience
  • Skill and expertise in EMIS Web
  • Experience of Docman and coding
Desirable criteria
  • Understanding of QOF and Primary Care targets
  • Understanding of PCSE work - Registrations and Deduction processes

Experience of summarising
