Do you have an interest and passion for Tissue Viability? Are you passionate about complex wound care assessment, planning and evaluating to improve patient outcomes, if yes then this could be the job for you.

An exciting opportunity has arisen to join the Ambulatory Care District Nursing Clinics.

Prif ddyletswyddau'r swydd

You do not need to have community nursing experience as we are committed to supporting your development during your induction and beyond. It would be an advantage if you have experience in vascular, surgical nursing and or tissue viability experience, although not essential.

We are a transforming service working towards enhancing care provision for people with complex wounds, surgical drains, catheter care and nephrostomy care. As an evolving service we have many service improvement initiatives in development which support the implementation of the National Wound Care strategy. We work collaboratively with the Tissue Viability Team, Vascular Service, Foot Health and Compton Lymphoedema service as well as with regional networks.

Within this role, you will independently run a variety of clinics, completing comprehensive holistic assessments, advising on optimal wound treatment and ensuring patient care plans follow the, catheter and wound care pathways.

  • Latest technical innovations to support wound care monitoring and healing
  • Support you in complex case management of patients
  • Offer professional advocacy sessions within house professional advocates for one to one and group sessions
  • As of 2024 we are introducing the specialist practitioner qualification course, to support the development of our junior workforce
