Job overview

We are currently looking for an experienced, knowledgeable and educated Advanced Clinical Practitioner to join our team on Adult Community Services - West Park. You will be able to work autonomously making high level decisions to treat health needs as part of the wider MDT.  

West Park Hospital is looking for an Advanced Clinical Practitioner to proactively manage those patients still in receipt of hospital based care, optimising their condition to support rapid onward transfer to the place they call home. There will be a requirement to facilitate ward rounds, huddles and undertake planned and unplanned clinical interventions to prevent a transfer back to the acute site.

The role will be integral to ensuring that West Park continues to be a vibrant community hospital that offers specialist rehab across the Black Country, and the successful candidate will need to work collaboratively with the ACS management team and West Park clinical teams to deliver this.

Main duties of the job

The post holder will be expected to exercise a high degree of personal professional autonomy and make critical judgements to satisfy the expectations and demands of the role and service. The post holder will support the Medical team in developing a new way of working that emphasises a more efficient and patient focused service and will ensure the safe referral and discharge of patients. This post is firmly grounded in direct compassionate care provision or clinical work with patients, families and populations.

To be professionally accountable and maintain the standards of professional practice as set by the appropriate regulatory body applicable to your profession or role.

Qualifications, Skills, and experience

Essential criteria
  • NMC Registration Nurse or HCPC registration
  • Master’s level education [Healthcare related] / Working towards
  • Health Assessment qualification
  • Experienced V300 Non-Medical Prescriber
Desirable criteria
  • Experience of autonomous practice in a secondary setting
  • Evidence of professional updating within last 12 months specific to sphere of practice
  • Knowledge of Rehab and Frailty patients
  • Worked in an acute setting
