Job overview

An exciting opportunity has arisen for a mutlidisciplinary Advanced Clinical Practitioner (ACP) (NMC/HCPC/GPhC) at Band 8a to join our Urgent Community Response Services Directorate within the Primary Care, Community, and Therapies Clinical Group. 

You will be supported by our Lead ACP, who works closely with the Director of Advanced Practice for the organisation, ensuring appropriate supervision, governance and fulfilling all four pillars of ACP. As part of this dynamic team, you’ll collaborate with ACPs, trainee ACPs (tACPs), enhanced practitioners, community matrons, and case managers. Your role will primarily focus on the UCR directorate, but you’ll also work across Primary Care and broader community services—key elements of our Trust’s commitment to delivering excellent and responsive integrated care.

Main duties of the job

Role Responsibilities

The successful candidate will be an autonomous practitioner, conducting independent health assessments for undifferentiated and undiagnosed conditions, specifically focusing on the nine Urgent Community Response (UCR) conditions.

Key Responsibilities:

  • Clinical Leadership:

    • Provide clinical leadership and support for trainee ACPs (tACPs) and the wider UCR workforce across the Primary Care, Community, and Therapies Clinical Group.
    • Play a key role in the development and supervision of advanced practice within the team.
  • Service Development:

    • Contribute to the ongoing development of UCR services, governance processes, and the establishment of high clinical standards.
  • Patient Care:

    • Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions, where appropriate.
    • Diagnose and manage both acute and chronic conditions, integrating drug-based and non-drug-based treatment methods into comprehensive management plans.
    • Prescribe and review medication to ensure therapeutic effectiveness, aligned with patient needs, evidence-based practice, national guidelines, and formulary protocols.

Qualifications

Essential criteria
  • First level registration with the NMC or appropriate allied health professional governing body
  • Master’s level education in relevant health related subject
  • Completion of recent Leadership /Management or evidence of equivalent competence
  • Evidence of contemporaneous CPD
  • Independent Prescriber qualification or working towards
  • MSc Qualification
  • Full UK driving license
Desirable criteria
  • Non-medical prescribing qualification
  • Full ECP (Injuries & Illness)

Other

Essential criteria
  • UK Driving Licence
  • Access to own vechile for work purposes

Experience

Essential criteria
  • Significant recent experience in a senior nursing or allied health professional role within a primary care or emergency care setting
  • Experience of leading clinical teams and professional agendaS
  • Experience of leadership role and change management
  • Proven track record in improving patient experience and gaining patient/public views
  • Recent experience of direct involvement in delivering the Quality Agenda
  • Experience of telephone triage
  • Experience of effective interagency working
Desirable criteria
  • Experience of multi-skilled approach to Chronic Disease Management
  • Experience in clinical systems, preferably System One / Emis.
  • Evidence of innovative practice
  • Experience of working within an ACP post within primary or community care
  • Experience of working as or towards autonomous practice
