Walsall Healthcare NHS Trust provides integrated acute and community Healthcare in the West Midlands serving a population of 260,000 residents. Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit was completed in 2018, extension of a Neonatal Unit and an approved plan to build a brand new, state of the art Emergency Department which commenced in 2021

We provide high quality, friendly and effective community health services from multiple sites covering Walsall. Our multidisciplinary services include rapid response and homebased care, so that those with long term conditions and the frail elderly can remain in their own homes.

Intermediate Care Services Team

An exciting opportunity has arisen for a qualified mental health Nurse to join the Intermediate Care Services (ICS) team at Walsall Healthcare NHS Trust.

 The post holder will form part of a multi-agency team supporting the hospital to achieve safe, timely and effective discharges from the ward areas and from the ICS pathway.

The post holder will be expected to work within the multi-disciplinary team to provide senior mental health nurse advice and expertise in relation to complex discharge planning. They will be responsible for completing comprehensive, holistic assessments and care plans to facilitate safe and timely discharge. The post holder will also support junior members of the team to promote quality and embed the ethos of the ICS.

The post holder will be expected to complete patient visits in the community post discharge in both patient's homes, Nursing and residential placements to plan for long term care and support.

The successful candidate will require baseline knowledge of Continuing Healthcare national framework as assessing for eligibility of such funding is a main requisite of the role.

The Successful candidate will monitor the patient's that require one to one care on discharge to a 24 hour care setting. 

The Successful candidate shall also monitor patient's that have delirium on discharge to a 24 hour care setting.  

Applicants should have previous post registration experience preferably on an acute ward and have experience and knowledge in discharging complex patients and CHC funding and pathways.

A knowledge of social services is also desirable to the post. Applicants must hold a registered mental health Nurse qualification. Candidates must be able to travel in the borough and surrounding areas and have access to a car for this purpose.

*Use advanced skills and expert knowledge to holistically assess patients of a defined client group, instigating therapeutic treatments, including extended and supplementary prescribing, based on best available evidence in order to improve health outcomes.
 To be responsible for initial and re-assessments for patients referred into the service.
 Support allocated G.P on weekly visits to nursing homes when appropriate to role .
 Attend weekly multi-disciplinary meetings to assist in discharge planning and arrange provision of ongoing care, to assist patients efficiently through their pathway.
 Organise and manage caseload demonstrating the ability to prioritise, on a daily basis.
 The post holder will be required to assess for Continuing Health Care (CHC) and Funded Nursing Care (FNC).
 Ensure plans of care reflect evidence based holistic and individual approach to care.
 Lead on planning discharge programmes with other agencies and encourage multidisciplinary working within teams.
 Assume responsibility for continuous professional development and personal knowledge including the maintenance of a professional portfolio and clinical competency.
 Promote the Trust’s clinical governance framework including participation in clinical supervision
