Job overview

The Palliative Care Support Team provide end of life care to patients in the last 12 weeks of life in their own home. The team is lead by trained nurses with clinical support workers who work  jointly with social care.

To  provide holistic care to  patients and their families/carers , delivering  high quality, empathetic end of life care

Working with the wider multi disciplinary team, developing working relationships with other team members and professionals

Ability to communicate both verbal and written skills

Flexible to meet the demands of the service

Main duties of the job

The post holder will work regularly without direct supervision whilst undertaking  duties assigned the team leader

Palliative Care Support Team is a joint service working with the local authority supporting patients at end of life enabling them to be cared for at home

Meeting daily with team, reporting back information from their patient visits to ensure consistency of patient care and that information supports the care of patients

Maintaining confidentiality while communicating patient information, in such a way that  preserves the dignity and privacy

Qualifications

Essential criteria
  • NVQ level 2 or equivalent in health and social care
Desirable criteria
  • NVQ level 3 or equivalent in health and social care
  • GCSE or equivalent in Maths and English language

Knowledge

Essential criteria
  • Demonstrate knowledge of duties required as a Clinical Support Worker
  • good written grammar and organisational skills
Desirable criteria
  • Computer literate

Experience

Essential criteria
  • Previous experience of caring for people in a care worker role within hospital, community or care home setting
  • Able to communicate to meet the needs of the role
Desirable criteria
  • Palliative care experience
