Job overview
A qualified Physiotherapist who is looking for the next opportunity as a Band 5 Physiotherapist within the community.
We are looking to recruit a motivated, enthusiastic and forward-thinking Static Band 5 Physiotherapist to join the Icares Care Home Team within Sandwell and West Birmingham NHS Trust.
This is a full-time role, 37.5 hours per week. Weekend and Bank Holiday working will be required. An exciting opportunity has arisen for a Band 5 physiotherapist to join our re-designed Community Therapy Service. (Includes Enhanced Assessment Beds both at Harvest view and at Care Homes at Sandwell)
Our multidisciplinary Community Therapy team provides a seven-day service:
· A supported discharge service to facilitate timely and safe discharges from our EAB units (both at Harvest view and Care Homes) to service users’ home or suitable permanent placement within the community.
· Short term support for people who require assistance with activities of daily living.
· A rehabilitation service, including a range of therapy support that includes comprehensive assessments, liaising with social workers, MDT discharges, access visits, equipment provisions etc.
· Community rehabilitation at Care Homes for people with long term conditions and frailty through spots purchase and permanent beds.
· · Discharge planning through MDT approach to facilitate patients to transfer the care from nursing homes to their own homes.
Main duties of the job
Due to the domiciliary nature of this post a full UK driving license and access to a car is essential. The post is subject to successful interview, references, DBS clearance and Occupational Health checks.
· · The post will involve working with patients with a wide variety of pathologies and medical conditions and will involve visiting services users in their own homes as part of post discharge access visit or in-reach to nursing homes.
· To establish effective communication networks with the service users, carers, multidisciplinary colleagues in health, social and care settings contributing to clear decision making with regard to treatment principles and discharge planning.
· To form professional relationships with service users / carers and communicate with them in a way that respects their views, autonomy and culture, and taking into consideration differences and difficulties.
· · To attend and feedback relevant information at team meetings, MDT meetings and case conferences as appropriate to the service user programme.
· · To liaise and make appropriate referrals to other community Icares Team and voluntary partners, to voluntary and statutory services in the Sandwell community.
· To be responsible for the maintenance of accurate, comprehensive and contemporaneous service user’s records detailing all Physiotherapy interventions and significant events, following Trust and departmental policies.
