An exciting opportunity has arisen for a clinically experienced pharmacist to join our highly respected team of integrated advanced specialist pharmacists and technicians. We are looking for an enthusiastic, self-motivated pharmacist that will provide clinical expertise and leadership, ensuring the delivery of a high quality clinical pharmacy service to care of the elderly wards, including our expanding virtual frailty wards.

You will be an integral member of the MDT, promoting medicines optimisation, providing expert advice and facilitating patient flow. The post holder will have excellent communication and strong interpersonal skills, ideally with an independent prescribing qualification or willing to work towards.

Main duties of the job

·         Provide an expert level of clinical pharmacy services to care of the elderly and frailty wards, including virtual frailty caseloads and ensure clinical provision to any new care pathways developed at The Dudley Group NHS Foundation Trust (DGH).

·         Lead specialist clinical pharmacy practice for the Older People directorate and be responsible for the delivery, development, review and co-ordination of clinical pharmacy services in line with standards defined by national priorities, divisional objectives and the DGH Pharmacy/Trust Strategy.

·         Responsible for all aspects of medicines management to optimise patient’s care involving the use of medication and aid in the delivery of the directorate’s key performance indicators

·         Actively participate in multidisciplinary ward rounds, including frailty virtual wards where appropriate, delivering and promoting high quality, cost-effective, patient-focussed pharmaceutical care.

·         To support medicines optimisation activities within the Care of the Elderly wards and providing support to the wider Multidisciplinary Team (MDT) in this area, when needed.

·     Development, implementation and audit of local guidelines and promote safe drug use within the specialty.

Qualifications & Education

Essential criteria
  • MPharm or equivalent academic qualification
  • Registration with General Pharmaceutical Council (GPhC)
  • Postgraduate Diploma in clinical pharmacy or equivalent experience/qualification
  • Independent prescriber and registered with the General Pharmaceutical Council (GPhC) or willing to work towards after appointment

Experience

Essential criteria
  • Experience of delivering clinical pharmacy service and advice relating to the safe use of medicines

Knowledge and Training

Essential criteria
  • Significant experience post registration and postgraduate experience in hospital Pharmacy

Communication & Relationship Skills

Essential criteria
  • Excellent interpersonal and presentation skills, both verbal and written to communicate highly complex drug or medicine related information.
  • Ability to work in a multidisciplinary team

Analytical & Judgement Skills

Essential criteria
  • IT literacy including the ability to use a range of Microsoft 365 packages, Power Point, email and use of the internet to research evidence-based medicine and to utilise medical databases.

Planning & Organisational Skills

Essential criteria
  • Evidence of planning, implementing and monitoring outcomes in research, quality improvement or audit
