An exciting opportunity has arisen for a clinically experienced pharmacist to join our highly respected team of integrated advanced specialist pharmacists and technicians. We are looking for an enthusiastic, self-motivated pharmacist that will provide clinical expertise and leadership, ensuring the delivery of a high quality clinical pharmacy service to care of the elderly wards, including our expanding virtual frailty wards. 

You will be an integral member of the MDT, promoting medicines optimisation, providing expert advice and facilitating patient flow. The post holder will have excellent communication and strong interpersonal skills, ideally with an independent prescribing qualification or willing to work towards.

Prif ddyletswyddau'r swydd

  • Provide an expert level of clinical pharmacy services to care of the elderly and frailty wards, including virtual frailty caseloads and ensure clinical provision to any new care pathways developed at The Dudley Group NHS Foundation Trust (DGH).  

  • Lead specialist clinical pharmacy practice for the Older People directorate and be responsible for the delivery, development, review and co-ordination of clinical pharmacy services in line with standards defined by national priorities, divisional objectives and the DGH Pharmacy/Trust Strategy.  

  • Responsible for all aspects of medicines management to optimise patient’s care involving the use of medication and aid in the delivery of the directorate’s key performance indicators 

  • Actively participate in multidisciplinary ward rounds, including frailty virtual wards where appropriate, delivering and promoting high quality, cost-effective, patient-focussed pharmaceutical care.  

  • To support medicines optimisation activities within the Care of the Elderly wards and providing support to the wider Multidisciplinary Team (MDT) in this area, when needed. 

  • Development, implementation and audit of local guidelines and promote safe drug use within the specialty.

Qualifications & Education

  • MPharm or equivalent academic qualification
  • Registration with General Pharmaceutical Council (GPhC)
  • Postgraduate Diploma in clinical pharmacy or equivalent experience/qualification
  • Evidence of commitment to continued professional development (CPD)
  • Independent prescriber and registered with the General Pharmaceutical Council (GPhC) or willing to work towards after appointment

  • Postgraduate Masters or doctorate in relevant subject or discipline
  • Member of the Faculty of the Royal Pharmaceutical Society of Great Britain or working towards
  • Management qualification relevant to the NHS

Knowledge & Training

  • Significant experience post registration and postgraduate experience in hospital Pharmacy
  • Broad knowledge base of a wide range of clinical areas
  • An understanding of the role and its application to patient care
  • An understanding of clinical governance, safety and risk management and its application to patient care
  • Able to train in and instil good prescribing practise and good clinical practice to members of the multi-disciplinary team including doctors, nurses and pharmacists

Experience

  • Experience of delivering clinical pharmacy service and advice relating to the safe use of medicines
  • Experience of training and education of both pharmacy staff and other healthcare professionals
  • Some experience of excellent leadership and management skills

  • Publication of audit/research/clinical work
  • Evidence of incorporating innovative and ‘best practice’ into current practice.

  • Able to communicate with patients and their relatives regarding complex prescribed medications providing both verbal and written communication and to motivate and reassure patients regarding their treatment
  • Able to identify barriers to communication (e.g. hearing, non-English speaking) and implement solutions to overcome said barriers
  • Confident and assertive; able to defend opinions in a constructive manner when challenged

Analytical & Judgement Skills

  • Able to critically appraise and evaluate information and communicate the findings clearly in simple terms

  • Ensure clinical practice is evidence based
  • Experience in making judgements involving highly complex facts, which require analysis, interpretation and options appraisal e.g. drug expenditure
  • IT literacy including the ability to use a range of Microsoft 365 packages, Power Point, email and use of the internet to research evidence-based medicine and to utilise medical databases.
