We pride ourselves on being both innovative and bold as we plan and deliver services to our populations.  This exciting opportunity will mean working alongside other members of the primary care team to support medicine optimisation within an extensive and diverse population. This will be done through various patient facing and non-patient facing clinics.

General Practice is constantly changing and it is an exciting time to join us and become part of our ever growing, supportive, and forward thinking Trust to support our local primary care networks.  

Main duties of the job

A key role will be involved in developing a strong relationship with our local care homes and through activities such as patient facing structured medication reviews, outlined in the NHS Network Contract Direct Enhanced Service document.  Clinical pharmacists are also key to improving access for out patients,  there are a significant no of  calls to the practice which are related to prescription or medication queries.  The potential to improve patient care and experience in this area is significant - we want you to help shape this change!

The successful applicant will need to share the same high quality values of Trust  we work with  work as part of a team which includes GPs, Advanced Nurse Practitioners, Nurse, Physician Associates, and Social Prescribers.

A strong interest in Diabetes would also be beneficial.

Experience

Essential criteria
  • Significant post-qualification experience.
  • Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate

Qualifications

Essential criteria
  • Independent prescriber
  • Completed or willing to enrol on to start 18-month training pathway (e.g. Primary Care Pharmacy Educational Pathway or Medicines Optimisation in Care Homes).
Desirable criteria
  • Clinical diploma

Communication/Verbal Skills

Essential criteria
  • Excellent written and verbal communication skills
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety

Knowledge

Essential criteria
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-- based healthcare.
  • An appreciation of the nature of GPs and general practices.
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Good IT skills
  • Able to obtain and analyse complex technical information
  • Able to work under pressure and to meet deadlines
  • Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers
  • Safeguarding and other mandatory training
