Referral Coordination Hub Clinical Nurse Specialist

The RCH Clinical Nurse Specialist will lead for the Referral Coordination Hub (RCH) bed management function within the WMCPC which includes, patient flow, escalation, system relationships and bed capacity management with the ultimate goal being to avoid any avoidable admissions, for service users being placed in quality assured local contracted beds and elimination of inappropriate OOA placements. 

As a key member of the senior team, you will be expected to provide senior leadership and management support to the RCH teams managing patient flow within the WMCPC

Provide a focal point for the day-to-day management of patient flow and flow and escalation issues, including bed management, supporting internal and external stakeholders to reduce waiting times, to ensure effective use of contracted stock, to lead and respond to escalations and to ensure robust processes are in place and followed in relation to flow (admissions and discharges)

Ensure that all digital systems are used efficiently and in real time to ensure relevant and accurate data is captured and reported on, patient flow systems are efficient, responsive, safe and are provided to the highest possible standards, in line with Trust values. You will use data effectively to ensure activity is forecast forward and empower a culture of doing tomorrow’s work today to improve flow.  

Support the bed managers and  referral hub team in the development, implementation, management and review of the WMCPC’s clinical and operational plans, policies and escalation protocols which support the management of patient flow and discharge in regard to use of beds. 

Deputise for the WMCPC leadership team  and/or represent the WMCPCin matters relating to the operational management of patient flow as required on ICS, acute, regional, provider calls as appropriate. 

Ensure patient bed activity is balanced to meet key access targets, including admission flow, discharge case management, CRFD’s’s and discharge flow working alongside referral hub management team and WMCPC case managers.

Here in the West Midlands - and across the country – those who need an inpatient admission experience either a long wait for a bed or are admitted to hospitals far from home.

We hope that by working collaboratively, using local data and listening and engaging with our service users, we will be able to reduce the number of young people admitted to inpatient services, drive down length of stay, bring care closer to home and ultimately improve the outcome and experience of every young person we see.
