We currently have an opportunity an 8a Case Manager specifically within the Adult Secure Care Case Management Team within the West Midlands.
The successful candidate will work within the Adult Secure Care Provider Collaborative – Reach Out Commissioning HUB based in Birmingham and support the substantive Case Management team in delivering a high-quality service within the adult secure care mental illness and personality disorder pathway.
The successful candidate will be responsible for maintaining oversight of a specific caseload of individuals within specialised secure hospital environments ensuring access to appropriate treatment, commissioning oversight, quality and governance and discharge planning development.
The role also requires working closely and collaboratively with other commissioning team members in Local Area Teams (LAT) advancing the collaborative commissioning agenda of NHS England/Reach Out.

You will be accountable and responsible to a Senior Case Manager who with the Clinical Program Director for Reach Out will be supporting you with your day-to-day responsibilities.
To provide clinical support to the specialist commissioners in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.
The role supports the business in driving transformation as well as value for money in planning, commissioning and service the role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery.
