The clinical role is based within Urgent Care/SPA supporting the clinical triage of new patient referrals and messages under the supervision of the Clinical Team Lead.  The post-holder will be accountable for the delivery of high-quality care to patients within their own homes and or other community settings, preventing avoidable hospital admissions. Working with other members of the wider team to provide high quality care and to promote health/well being and independence for patients on the team caseload. As part of this role rotation to the Early Intervention Care Teams across the city will take place every 4-6 months.

 

Main duties of the job

          Work in accordance with the NMC Code of Professional Conduct, scope of Professional Practice, Trust Policies and practice in an evidence based manner.

 

          In conjunction and under the supervision with the caseload holder, take responsibility for the assessment, planning, implementation and evaluation of allocated patients needs.

 

 

          Identify risks through the assessment process documenting and initiating appropriate action, reporting as appropriate to the caseload holder.

 

       

 

      To act as an autonomous practitioner providing comprehensive telephone triage and clinical assessment service to GP/patients/carer resulting in a safe and appropriate clinical decision. Given the complexity of patients being referred through the SPA, there will be barriers to understanding, the post-holder will also deal with very emotive and sensitive situations in regards to patient care.

 

 

         

       Work in partnership with GP’s, Health and social care and other agencies, both voluntary and statutory following patient pathways to provide comprehensive packages of care.

 

 

 

 

      Recognise deterioration in patient’s condition advising caseload holder of any circumstances which give cause for concern.

 

     To maintain accurate records of all clinical consultations and ensure that the NMC Code of Conduct is adhered to.

Detailed job description and main responsibilities

 

1.    Maintain own professional development in line with KSF requirements, undertake training and obtain competency in any new areas of care that are identified by service need.

 

 

2.    Act as a role model, assessor and mentor to students on placement. Provide community experience and education for visitors to the Trust and participate in reflective practice within the team.

 

To undertake other duties commensurate with this grade of post in agreement with the relevant line manager.

 

To minimise the Trust’s environmental impact wherever possible, including recycling, switching off lights, computers, monitors and equipment when not in use. Helping to reduce paper waste by minimising printing/copying and reducing water usage, reporting faults and heating/cooling concerns promptly and minimising travel. Where the role includes the ordering and use of supplies or equipment the post holder will consider the environmental impact of purchases.

 

 

Key Relationships

Establish and maintain effective communication pathways with internal and external stakeholders (eg GPs, hospital clinicians, local authority).
