Look no further, we have an exciting opportunity for an experienced Senior Nurse to join our Early Intervention Community Team within Birmingham Community Health Care NHS Foundation Trust, the largest community trust in the country.

Part of a wider Early Intervention programme, which has so far delivered: 77000 fewer acute bed days used, 19000 fewer non-acute bed days and people spend 11.5 fewer days in the system.  Overall having an impact of £25.8million saved for Birmingham to reinvest in Health and Social Care developments.

You will be joining a community team in the midst of one of the largest Health and Social Care transformations in the country, with exciting changes and developments, preventing avoidable hospital admissions, and facilitating early hospital discharge, plus the added bonus of Birmingham being a fantastic place to work and live.

Carry out comprehensive holistic assessments of the care needs of people referred to the service seeking support and advice from senior colleagues in situations that are outside the post holder’s sphere of competence.

With support where necessary from senior colleagues, plan, implement and evaluate programmes of care designed to maintain the patient in his/her own home.

In situations where the patient cannot be maintained safely at home facilitate admission to the most appropriate setting.

Actively promote participation by the team in current quality initiatives for example, falls prevention.

Manage own time effectively and prioritise workload.

Facilitate teaching and supervision for less experience members of staff.

Promote health by collaborative working with patients and their relatives.

Demonstrate teach and supervise basic nursing care that families can safely provide between visits.

In conjunction with therapist on the team, measure for and order special adaption’s and equipment indicated by the assessment and plan of care.

Qualifications/training

  • Registered Nurse Level 1 (Adult)
  • Evidence of up to date post registration education relevant to the role.

  • Mentoring or teaching qualification.
  • Degree in relevant subject
  • Nurse prescriber

Experience

  • Significant experience as a registered nurse including working in a rehabilitation setting or care of the older adult.
  • Experience working as a registered nurse in the community.
  • Significant experience of a multidisciplinary approach to care.

  • Competent in IV drug administration and cannulation.

  • Skills to organise team workload and prioritise.
  • Awareness of legislation affecting the role including equal opportunities law.
  • Ability to evidence application of utilising evidence-based practice.

  • Comprehensive assessment skills and knowledge of the needs of the older person including risk management.
  • Computer literacy skills.
  • Knowledge of current legislation, particularly those affecting community services.
  • Knowledge and application of the NMC code of Conduct and Scope of Professional Practice.
  • Understanding of the principles and application of Clinical Governance.
  • Knowledge and understanding of clinical audit.
  • Excellent verbal and written communication skills.
  • Understanding of the complex needs of a socially and culturally diverse population.
  • Understanding of legislation related to Safeguarding Vulnerable adults and children.
