Band 3 Stomatherapy Administrator / Assistant Pathway Coordinator

This is a part-time post for 16 hours per week.

The postholder will take the lead in providing dedicated administrative support to the stoma care nursing team. 

Duties include:

Maintain effective office procedures and processes.

Online and paper diary management, uploading of weekly activity data (surgical tracker), booking of patient’s clinics and outcomes on to the hospital system, formatting and sending out confidential clinical letters and liaising with patients in a professional manner.

You will become an expert user of our patient database.

You will be responsible for departmental ordering, using online procurement and ordering stoma supplies. 

Assist in planning meeting and product accurate minutes and use Microsoft Office skills. 

Maintain effective communication across the team, taking phone calls and documenting in mime and EPR. 

You will be proactive in dealing with enquiries into the service and be comfortable working independently at times.

Main duties of the job

The postholder's key relationships will be with:

Internal: Stoma care team, outpatients departments, ward clerks, departmental secretaries (including private), procurement, systems and IT support and other members of the multidisciplinary team.

External: Patients, GP surgeries, community nurses,other hospitals and Dispensing Appliance Contractors (DACs) product manufacturers and their representatives.

Detailed job description and main responsibilities

To ensure that clinical activity is booked to patient records (EPR), and that patient outcomes are accurately coded.

To support the team in the production of regular audits reports to allow monitoring of departmental activity.

To inform dispensing appliance contractors (DAC) of patients discharge requirements and answer any queries.

To ensure all patients details are correct on the patient’s database and EPR.

Assist with planning and arranging meetings and in the taking of accurate minutes and ensuring timely dissemination

Adding new referrals from the surgical tracker.

Putting new referrals on to EPR and liaising with the stoma team regarding organising appointments.

Adding new referrals and patients to the database system, updating interventions and recording deaths or changes in circumstances

Communicate with other hospitals regarding sending and receiving referrals for patients out of area.

Contact patients offering them a choice of clinic dates, including face-to-face or telephone clinics.

Assist in the management of the appointment diary and the message diary, ensuring all calls, appointments and home visits are logged on EPR in a timely manner.

Use IT systems as required.

Comply with all mandatory training.

In line with departmental protocols communicate basic clinical information to patients relating to their procedures and treatment ensuring appropriate distribution of patient information.

Use judgement and initiative to prioritise the importance/urgency of particular requests, bleeping stoma nurses when emergency calls come through.

To provide a secretarial service to the Stoma Care Nursing Team as agreed, whilst adhering to codes of practice concerning confidentiality and Data Protection.

Word process letters and other correspondence as required (clinic letters; discharge letters to GPs and District Nurses using predefined templates).

To be able to contribute to speciality improvement and efficiency processes.

Book preoperative assessment appointments with the agreed timescales.

Manage your own workload and prioritise your workload.

Maintain good levels of stock for the stock cupboard, order departmental supplies on a weekly basis.

Carry out any other reasonable duties commensurate with the role and delegated or requested by the stoma team.
