Surrey Downs Health & Care

The post holder is responsible for the continuing management of the twilight patients and/or co-owners within their clinical setting, without supervision. This includes the assessment of care needs, the development, implementation and evaluation of programs of care, the setting and
maintaining of standards and the supervision and deployment of the twilight team, if appropriate.
The post holder will undertake the facilitation of educational programs for basic, post-basic and in-service training, in close liaison with the Learning and Development Team. To work closely with the Community Hub teams to facilitate patient-focused, innovative research-based practice, in
line with Surrey Downs Health & Care objectives. To promote continuity of care between twilight and day services through effective communication and processes. To coordinate closely with all Surrey Downs Health & Care twilight teams to ensure a joined-up (seamless) service is delivered
and the teams are supported.

Main duties of the job

Patient Care and Assessment:

  • Provide skilled nursing care, assessing needs, and creating, implementing, and reviewing care plans.

Detailed job description and main responsibilities

As a B6 Senior Staff Nurse in the Twilight Team, your primary responsibilities will include providing high-quality, skilled nursing care to patients, assessing both their immediate and long-term needs, and supporting their families and carers. Your role will involve leading the team, ensuring adherence to policies and procedures, and maintaining professional and clinical standards. Below is a detailed outline of your main duties:

Patient Care

  • Skilled Nursing Care: Deliver comprehensive nursing care, assess patient needs, and develop, implement, and review individualized care plans.
  • Patient Dignity: Treat all patients as individuals, respecting their privacy and dignity at all times.
  • Family Support: Involve, inform, support, and educate patients' families and carers as appropriate.
  • Health Promotion: Actively promote patients' health and well-being through education and health interventions.

Team Leadership

  • Staffing and Cover: Plan and ensure appropriate staffing and skill mix, guaranteeing adequate cover at all times. Report any staffing issues to the operational manager.
  • Meeting Coordination: Attend meetings with other Band 6 twilight colleagues to coordinate the management of the twilight service, including the formulation of robust off-duty schedules.
  • Shift Leadership: Work towards ensuring one Band 6 nurse is on duty across all twilight teams each evening.

Documentation and Compliance

  • Record Keeping: Maintain clear, comprehensive, signed, and contemporaneous records in line with Surrey Downs Health & Care procedures.
  • Policy Adherence: Ensure that all policies, procedures, and standards of care are adhered to consistently.
  • Supervision and Mentoring: Act as a mentor and assessor to both qualified and unqualified staff, supporting their professional development.

Professional Development

  • Knowledge Update: Keep up-to-date with all aspects of clinical practice and act as a resource for other staff members.
  • Teaching and Training: Plan, participate in, and evaluate teaching programs for all staff levels in collaboration with the senior team and Learning and Development Team.
  • Training Needs: Identify and address staff training and development needs, fostering a positive clinical learning environment.

Operational Responsibilities

  • Resource Management: Work within the allocated budget and ensure the economic use of resources. Participate in the recruitment and selection process when needed.
  • Clinical Governance: Participate in the Surrey Downs Health & Care Clinical Governance Strategy, facilitating clinical audits and monitoring care quality.
  • Risk Management: Adhere to the Surrey Downs Health & Care Risk Management Strategy, ensuring all protective measures for vulnerable individuals are followed.

Additional Duties

  • Professional Integrity: Maintain your professional and clinical integrity in line with NMC guidelines.
  • Multidisciplinary Approach: Ensure the effectiveness of care through a multidisciplinary approach, collaborating with other healthcare professionals.
  • Flexible Duties: Undertake other duties as required, consistent with the responsibilities of the post.
