We have an opportunity for a Nurse Associate or Assistant Practitioner to join our team.

This role will complement the work of the Wound Care and Lymphoedema Service specialist nurses to improve patient experience and outcomes. T

he role supports the specialist nurses in providing comprehensive assessment, treatment planning and evaluation providing a unique interface supporting the integrated network teams.

Prif ddyletswyddau'r swydd

To work with the Sirona Wound Care Service across the organisation supporting the integrated network teams in vascular assessment using a Doppler ultrasound.

To work with the Lymphoedema Service to use shared decision making to empower and support patients and carers with their short- and long-term self-management plans.

To work independently as part of the team, providing specialist wound care, lower limb assessment and lymphoedema care following guidelines and protocols.

Liaison with the specialist nurses following assessments to plan treatment based on results.

Be able to and willing to work flexibly and to travel throughout the Sirona area

  • Successful completion of Foundation Degree Level 5 or equivalent or willing to undertake
  • Evidence of continuous personal/professional development relevant to the role
  • Willingness to undertake training required for the role

Skills, experience and knowledge

  • Able to carry out relevant clinical competences required of the post implementing and evaluating care, using agreed protocols and reporting adverse signs to registered professionals
  • Contribute to the holistic assessment of the service user
  • Excellent communication skills in varied ways with colleagues, service users and other stakeholders
  • Able to use a variety of IT platforms i.e. EMIS, email
  • Experience of direct client care in health or social care setting
  • Experience of team working and working without direct supervision

  • Up to date knowledge of Evidence Based Practice, local and national standards
