Job overview

We are looking for an motivated and committed individual with interest in inpatient rehabilitation who will be part of a supportive therapy team  delivering  therapy within South Bristol Community Hospital.  The rehabilitation unit meets the needs of people who are unable to return home immediately after a hospital stay, as they need further  rehabilitation following an acute illness/injury.  

The unit consists of 30 beds at South Bristol Community Hospital, which provides a purpose built environment where individuals who are ‘medically stable’ can receive rehabilitation assessment, along with focused therapy and support from our multidisciplinary team, to support independence before they return home. 

We are looking for a Rehab Support Worker to work with the therapy team to deliver rehabilitation. You will be working with experienced health care professionals as part of an integrated multidisciplinary team who will give support, joint working, regular supervision, appraisal, competencies and  training programme and a real commitment to professional development for you to reach your full potential.

Main duties of the job

The post-holder will undertake therapy lead activities within the rehab unit. They will be part of the multi-disciplinary team providing rehabilitation and care. This includes supervising patients with personal and domestic living activities, mobility practice and therapy programmes.

 To be able to undertake a range of suitably delegated therapy led activities within a competency framework to provide therapy to patients 

 To contribute to the assessment  and treatment of patients referred to the inpatient rehabilitation unit. Following prescribed aspects of care, supervising patients with personal and domestic living activities, adaptions of mobility aids and the provision and fitting of basic aids.

To assist in the running of group treatments, including leading on independent treatment groups as prescribed by the treating therapists.

To maintain accurate, legible, and contemporaneous service patients’ records (paper/electronic) in line with documentation guides.  

To provide a safe, patient centred, effective and evidence-based clinical activities in the inpatient rehabilitation setting

To assist therapists in the collection of accurate data for audits

To communicate effectively with patients and other stakeholders verbally, in writing and electronically using E-mail. This includes writing clinical records in line with the Sirona care & health Documentation Policy

Essential criteria
  • NVQ level 3 in Health and Social Care or equivalent

Experience

Essential criteria
  • Experience of direct client care in a health or social care setting
  • Experience of team working
  • Able to communicate clearly and succinctly, verbally, in writing and electronically
  • Experience of use of IT systems
  • Able to carry out the core clinical competencies required of the post e.g. manual handling of patients
Desirable criteria
  • Experience working in community, acute settings and with therapists
