Job overview

Sirona's Community Homeless Health Team supports individuals who are homeless, sofa surfing and in temporary and emergency accommodation.  We help 'bridge the gap' and promote re-engagement with health care.

Main duties of the job

Day to day, you will contribute to tackling health inequalities by working with the most vulnerable groups. 

You will proactively engage with individuals who have identified health inequalities at a range of venues in local communities, helping them to improve their lifestyle and overall health.

You will be an experienced practitioner who, acting within their professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.

Working as part of the team and as an autonomous practitioner, you will be self-driven demonstrating safe, evidence based clinical decision making to deliver compassion expert care for patients who are homeless.

You will work collaboratively with other agencies including health and social care professionals, specialised voluntary staff and Police to meet the needs of patients, supporting the delivery of policy and procedures, providing nurse leadership as required.

The role demands a unique practitioner with exceptional communication skills and an ability to adapt to challenging situations. You will need to quickly develop trust with this cohort of patients, encouraging them to access and utilise and engage with service for their often complex needs.
