Do you want to be part of a new, emerging service who are passionate about preventing hospital admissions, supporting early hospital discharges and delivering personalised care in the comfort of peoples homes? In keeping with the new developing transformation change to using virtual ward models, Sirona care & health are collaborating with UHBW & NBT NHS Trusts to deliver the NHS @ Home service to patients across BNSSG (Bristol, North Somerset & South Gloucestershire).

As a team we are blending medical innovation with bespoke care, bridging the gap between hospital and home. A virtual ward is a safe and efficient alternative to NHS bedded care that is enabled by technology and home visits. This innovative approach enables us to deliver safe, high quality and personalised care to people at home where they would rather be.

As a Senior Clinician with NHS@Home you will be:

  • Deliver community NHS@Home service including providing community visits, supporting virtual monitoring, coordination of clinical care.
  • Support professionals with advice and guidance as appropriate.
  • To provide both planned and unplanned services .

Our current operating hours of the service: 8am-8pm 7 days per week. We are open to discussion regarding shift patterns and hours of work at interview.

Please note: You must have a driving licence & use of your own car for work purposes for this role. This is not a remote position.

Main duties of the job

The NHS @ Home Service  is a new and exciting development within the NHS and this post offers the opportunity to help establish and develop the BNSSG NHS @ Home Hub to support both early discharge of inpatients and admission avoidance. By using bespoke software, patients are monitored remotely using a variety of technology options and a daily patient symptom questionnaire. Patients still benefit from the visits of our passionate clinicians when required and this role is not limited to virtual working.

As a Senior Clinician working in the NHS @Home service you will provide day to day management of caseload and staffing, alongside the hands on elements of carrying out both home visits and virtual monitoring to support patients across BNSSG.

Key responsibilities:

 

  • To support the delivery of community visits and remote monitoring (@Home) service.
  • Provide day to day management of caseload and staffing
  • Deliver direct care to patients in their own homes as part of a team

Flexibility, willingness to learn and a 'can do' attitude will be essential in this exciting new seven-day service. This post is open to all nursing and AHP staff with relevant experience - training in the use of remote technology will be included within the induction of the post.

  • Experience of undertaking specialist triage and remote assessments of patients, including those who may have complex presentations, multiple pathologies or high anxiety due to an acute condition
  • Experience and ability of supervising staff
  • Experience of communicating across organisational boundaries primary, secondary care and social care services
  • Knowledge of clinical governance processes (eg Audit)
  • Knowledge of shared decision making and goal setting, ability to implement this within patient care
Desirable criteria
  • Experience of digital consultations
  • Experience of leading and facilitating teaching for professional groups
  • Experience of community care
