We have an exciting opportunity for a Ward Discharge Co-ordinator to join our friendly and dynamic team at Bishops Castle Community Hospital. 

The role involves working within the ward environment as part of the multidisciplinary team, to provide a continuous coordinated approach to the safe and timely discharge of patients with simple and complex discharge needs.

Prif ddyletswyddau'r swydd

  • Communicate and liaise with patients, their families and carers and other key external stakeholders who play an integral role in patient discharge. 
  • Support the delivery of the SAFER red to green approach to expediting patient care.  
  • The role requires a high proportion of autonomous working with in the ward team within the sphere of the role.  
  •  Attend ward round, organise and chair MDT meetings and take action to expedite investigations and treatments to support pathway progression using the SAFER principals of red to green. 
  • Liaise with other members of the wider interdisciplinary team i.e. social services, ICS, district nurses, specialists nurses and other relevant external agencies to facilitate timely and co-ordinated patient discharge.

Experience

  • Working with hospital computer systems.

Knowledge and Skills

  • Knowledge of the Red to Green concept for discharge.
  • Articulate with well-developed communication (verbal and written) and interpersonal skills.
  • Computer literate with experience and knowledge of Microsoft Office - Word, Excel, PowerPoint and Outlook.
  • Good organisational skills; self-starter and able to use initiative appropriately with only little supervision.
  • Ability to prioritise and work on own initiative.
  • Demonstrates accuracy and attention to detail.
  • Professional and pleasant telephone manner.
  • Demonstrated appropriate level of problem resolution skills e.g., workload prioritisation, diary scheduling.
  • Excellent customer service skills, understanding the demands of a client focused organisation.
