If you are a registrant who is motivated and passionate for the highest standards of care read on.  This is a very exciting opportunity to join the Trusts Learning Disability Liaison Team.  The team works across the Trust (children, young people and adults) supporting clinical teams with expert knowledge advice and expertise in the care of people with a learning disability.  This includes supporting the planning process for elective admissions, inpatients and supporting the discharge process and advising on appropriate reasonable adjustments.  You will also be required to monitor acute admissions and support as required.

You will be expected to be a visible presence on the wards working closely with staff and patients and their families.  The successful candidate will work closely with the Trust MCA Lead Practitioner, clinical teams across the Trust, colleagues in community and primary care under the supervision of the Band 7 and also contribute to the LeDeR process.

The successful candidate will be a  Registered Nurse/Registered Allied Health Professional with current registration with a Diploma plus specialist qualifications or equivalent experience within specialist area.   You will act as a link to patient, carer, trust staff and external agencies.  You will support enabling patients with a learning disability to attain best possible access to Trust services ensuring all reasonable are considered to achieve this.  To advocate on behalf of the Trust’s patients to promote a positive experience of care.

  • Contributes to the facilitation of the planning, delivery and evaluation of care pathways for a designated group of patients.
  • Support the development of practice within the specialist field and contribute to the body of knowledge.

  • • Registered Nurse with current NMC registration/HCPC with current equivalent registration
  • • Diploma plus specialist qualifications / experience within specialist area; prepared to take degree level study
  • • Meets Nursing and Midwifery Council requirements for mentorship if relevant to role/ HCPC requirements for mentorship if relevant to role
  • • Evidence of ongoing continuous professional development.
