This post provides an unparalleled opportunity for senior trainees in Paediatric Intensive Care Medicine to further their training in all aspects of Paediatric Cardiac Intensive Care with an emphasis on heart failure, mechanical support and transplantation. There are research opportunities in the fields of heart failure and mechanical support. The post would be most suitable for trainees who are close to completion of training in PICM or indeed post CCT; in particular those would like to pursue a career in Paediatric Cardiac Intensive Care.
The start date for this post is April 2025 and would run for a year with potential for extension to a second year.
As well as the rapidly developing fields of cardiac transplantation, ECMO and mechanical support in this supra-regional centre; the Paediatric Cardiac Intensive Care Unit cares for the full range of congenital cardiac operations for our regional population.

Main duties of the job
This post provides unparalleled training in all aspects of paediatric cardiac intensive care including cardiac transplantation, ECMO and mechanical support. In conjunction with the PICU at the Great North Children’s we are accredited for training by the Intercollegiate Committee for Training in Paediatric Intensive Care Medicine (ICTPICM) and the Royal College for Paediatrics and Child Health.

Detailed job description and main responsibilities
The post will be based on the Paediatric Cardiothoracic Intensive Care Unit at Freeman Hospital with responsibility for outreach to paediatric cardiology patients on the Ward and 6 bedded HDU when on call. On PICU the post holder is responsible for a 12 bedded quaternary Paediatric Cardiac Intensive Care Unit. The rota is worked as a full shift system with out of hours 2nd call responsibility for the Paediatric Cardiology Ward. Full consultant supervision and support is provided. Whilst the unit throughput is primarily cardiac with over 300 cases per year it delivers respiratory ECMO and has done so for more than 20 years. It is one of the 2 UK Paediatric Cardiopulmonary Transplant Centres. There is also an active mechanical support programme for children with end stage heart failure. We have the second biggest Berlin Heart programme in the world and significant experience with other temporary and durable mechanical support devices. There is an active research programme with specific interests in mechanical support as a bridge to transplant, infant respiratory failure and coagulation management. The post holder will be encouraged to carry out a research project / audit during their time in post and to present this at a national / international meeting. The post holder will be responsible, in due course, for organising the departmental ECMO/mechanical support meetings and take an active role in the related teaching programme.
On-call Commitments:
The rota is worked as a 48-hour week full shift system. It is a hybrid rota 9-person rota incorporating trainees from Paediatric Cardiac Anaesthesia (2), Paediatric Intensive Care (5) and Advanced Paediatric Critical Care Practitioners (2). A Paediatric Cardiology Registrar is 2nd on for Paediatric Cardiology with Resident SHO, so the role of the PICU trainee on the Paediatric Cardiology Ward/HDU is for outreach support and review of deteriorating patients. Daytime work is based on PICU.
Training and Research opportunities:
In addition to clinical exposure to all aspects of Paediatric Cardiac Intensive Care Medicine and adopting a senior role when on clinical shifts, there are plenty of opportunities for research in the fields of mechanical support and heart failure. These ventures will be actively supported by the department and will be factored into clinical working pattern. Successful applicants will be expected to take part in the departmental mechanical support and CHD research meetings. There is now a mobile ECMO service in the region run from our department with potential for trainees to become involved. There are opportunities to attend theatre and echo clinics. Educational meetings comprise a full day every month of protected teaching in PCICU and additionally there is a programme of Cardiology and Cardiac Surgery teaching.
Development of non-technical skills is supported by opportunities for the senior trainee to lead ward rounds, handover, communication with other teams and bed management.
Flexibility:
In line with the Trust’s core value of placing patients at the heart of everything we do, we are developing our service provision to be responsive to the needs of our patients. To meet those needs some staff groups will be increasingly asked to work a more flexible work pattern so that they can offer services in the evening or weekend. As a result any offer of employment to a consultant post will be subject to you agreeing to work a new more flexible pattern or working in the future if required, including evening and weekend work.

Education and Qualifications
Essential criteria
- Primary Medical Qualification
- MRCPCH / FRCA or equivalent. Satisfactory completion of training to minimum of ST6 level or equivalent.
- Evidence of self-reflective practice
- Full GMC registration or eligibility to obtain this
- 12- 18 months training in general paediatric intensive care, including 6 months of paediatric cardiac intensive care
Desirable criteria
- Intercalated BSc or equivalent. Higher degrees including MSc, PhD or MD (where research thesis not part of full medical degree).
- Experience of paediatric cardiology.
- APLS provider or equivalent
- Wide paediatric / anaesthetic experience and neonatal intensive care training
- Within 12 months of completion of training in paediatric intensive care medicine
