The Crisis Resolution Intensive Support Service (CRISS) is a dynamic and flexible service which offers 24/7 crisis assessments and home-based treatment for working age adults across the city.  The service is currently under a period of development to bring together both sides of the service in localities across the city and in line with Core Fidelity standards. We’re looking for skilled senior practitioners who can help shape the service going forward.

We’re looking to recruit permanent Band 6 qualified practitioners to join our CRISS service. 

The successful Occupational Therapists/Mental Health Nurse or Social Worker will work across the service undertaking crisis assessments and home-based treatment.

As a band 6 practitioner you will operate within a community setting and will be required to work within a MDT led team which aims to reduce admissions to hospital and facilitate early discharge through home based treatment.   The role involves undertaking crisis assessments and implementing home-based treatment interventions. You will also work closely with inpatient and community services, providing an excellent alternative to inpatient admission and supporting in transitions points for services users to community services.

·                     To work closely with the Clinical Team Manager (CTM) in overseeing efficiency and effectiveness of treatment given and support the CTM in carrying out his/her duties.

·                     To present as a positive and professional role model to all staff within the team, striving to maintain a philosophy of service user involvement in continuously improving the service 

·                     Band 6 practitioners also provide leadership, management, and supervision / appraisal for junior staff (and sometimes other members of a multi-disciplinary team) and often lead / coordinate / supervise teams of staff in the delivery and evaluation of care.

·                     In some roles specialist skills and knowledge may be required. Band 6 practitioners will often have designated areas of practice to lead within the team. 

·                     Band 6 practitioners will be required to regularly take charge of a team and to delegate and supervise the work of other staff.

·                     Practitioners are expected to consistently demonstrate the ‘Six C’s’ – namely care, compassion, competence, communication, courage and commitment.

·                     Practitioners will be expected to work shifts over 24/7 pattern.

This role offers the successful candidate the opportunity to develop and provide leadership to the wider team whilst working clinically. You will actively participate and provide management and clinical supervision to other members of the team. You will be supported in your professional development to reach your potential in this exciting role. 

It is essential that applicants hold a full driver’s license and have access to vehicle for work purposes and are able to undertake night shifts.

· Communicates in an effective manner with all service users and carers, ensuring use of additional services where required, e.g translation services, communication aids, visual aids.

  To work as an integrated member of the clinical team, participating in all aspects of day to day service provision. 

·                     Band 6 practitioners work with a shared caseload of service users and as part of a multi-disciplinary team, providing same day/urgent assessment, care planning and care delivery. This will include the provision of highly skilled nursing/AHP care, psychological interventions, clinical assessment, risk assessments / risk management, and promoting recovery and inclusion.

· To provide advice, support and information to service users, other professionals and carers.

· Promotes effective team working to provide high quality care through effective communication and liaison.

· Ensure effective communication with the team and more senior team members during the planning, implementation and evaluations of care

· Liaise with other agencies, providers, service users/carers during the planning, implementation and evaluation of care as appropriate to the role. To establish and maintain close and effective relationships with inpatient and CMHT colleagues.

· Maintains effective communication and liaison systems which ensures the accurate and timely distribution and receipt of information (on a shift by shift basis) and is able to delegate responsibilities and duties where necessary.

. Manages and makes judgements regarding clinical work on a day to day basis, and is able to effectively coordinate a shift as appropriate

· Is aware of the limitations of their own role and utilises senior support and advice systems (including 24hr management advice structure) as required. May provide clinical supervision to colleagues if approached

· Attend local team and clinical meetings as required. Attend care group meetings as required and provide feedback as appropriate.

· Participate in managerial supervision and participate in appraisals with an identified supervisor.

. To provide clinical and managerial supervision, appraisal and other staff developmental work for junior staff and to act as named line manager for staff as required

· To adhere to all professional and Trust policies including the reporting of Trust incidents, complaints procedure and information governance.

 · In the role of senior clinician to assist and support staff from other areas with verbal and non-verbal complaints in a timely and effective manner, communicating outcomes to relevant parties, including mediating between the relevant parties.

 · Adhere to the Lone Working policy as appropriate

· Actively promote and support a client centred approach to care, founded in a philosophy of service user involvement.

· Develop and follow integrated Care Plans and document Safety Plan with individual clients and their family and carers, ensuring that the service user, and where appropriate carer, has an up to date copy of their plans.

. Post holder will be involved in the risk assessment of individuals (in conjunction with other members of the team, service users and their carers). They will ensure appropriate plans of action are in place to manage identified risks

 · Will maintain and produce detailed contemporaneous and accurate records and reports, in accordance with agreed procedures. Will possess IT literacy skills and have the ability to work with electronic care records.

To use professional judgement and exercise professional accountability in all aspects of clinical practice.
