We are pleased to be able to offer an existing opportunity for a band 6 Senior Community physiotherapist role working in the Reading area. The post holder is a Senior Practitioner with the clinical skills and expertise and knowledge to support community reablement /community Physio/ Falls service and 2-day UCR pathways. This is an exciting time to join BHFT, as we develop services to work in an integrated way within the community in reading and across Berkshire West.
The successful candidate will be required to provide high quality, responsive services working as an integral member of the intermediate care team and wider Integrated Community team. You will work closely with social services and voluntary organization for adults with a wide range of physical impairments and mental health conditions.
The successful candidate will work autonomously under the guidance of the team manager (as required) having a clinical responsibility for a designated caseload as well as supporting the clinical team with additional responsibilities.

- To post holder to mange the timely assessment and management of referrals with multiply co-morbidities and complex rehabilitation needs.

Support all intermediate care pathways to promote the timely assessment and management of referrals with multiply co-morbidities and complex rehabilitation needs.

To support more junior members of the intermediate care team with clinical development and caseload management.

To act as lead to triage the referrals for intermediate care services

Provision of equipment to promote independence and safety.

Hold a caseload and work autonomously

Work flexibly to ensure urgent referrals are adequately managed

Supporting colleagues in the delivery of compassionate, safe and structured care

Provide guidance and appropriate delegation within the team

To support the team lead/ team manager and participate in service development and quality improvement.

The post holder will hold responsibility for the provision of education and clinical physiotherapy advice to staff /students in Intermediate Care.

Previous Experience

- Experience of liaising with Social Care in the UK
- Experience of providing a broad range of interventions for a complex caseload

- Experience of working with acute and chronic conditions.
- Evidence of training in rehabilitation of older people.
- Training in the equipment needs of older people.
