The post is based within our main hub in Slough.

As an advanced mental health professional you would be directly supporting the triage team to make informed decisions regarding patient care for all new referrals into the team.  

The ideal candidate will be someone who has worked in an acute health care setting at Band 6 and feels ready for a next level challenge. They need to be motivated to work in a high demand and clinically exciting service. The ideal candidate will be someone who truly values the opportunities that the mental health professionals have in assisting people going through difficult times in their lives They will be passionate about helping people with SMI getting back on their recovery journeys following relapses.  

The team is very supportive with many very experienced practitioners from different clinical backgrounds.  

Prif ddyletswyddau'r swydd

The post holder will triage calls and NHS 111 referrals from professionals, patients and their carer’s into the CRHTT. Sign posting to appropriate services or opening to CRHTT and allocating workload as deemed appropriate. The post holder will direct and support the assessment of new patients to the service including risk assessment and the formulation and delivery of plans of care.

Education/Qualifications/Training

  • Registered Mental Nurse or Occupational Therapist or Allied Health Professional or Social Worker, whereby the person is licensed to assess, plan, implement and evaluate care for patients with mental health problems.

Previous Experience

  • Experience of clinically supervising junior and unqualified staff and mentoring students
  • Substantial experience of managing a busy caseload or service users with a range of mental health conditions
  • Experience of working effectively in a multi-disciplinary team and multi-agency environment
  • Strong knowledge and understanding of the health and social care systems and operating frameworks, clinical observations and the Mental Health Act

Knowledge, Skills & Abilities

  • Excellent knowledge and understanding of the health and social care systems and operating frameworks, clinical observations and the Mental Health Act
  • Previous knowledge and experience of working in a care environment and with service users who have complex mental health needs
  • Specialist knowledge acquired through training in a specialised discipline
  • Effective and competent interpersonal, influencing, communication, observation and reporting skills
  • Strong organisational skills and the ability to analyse situations and be able to make the right decisions
  • Able to empathise, be supportive and sensitive to the needs of others as well as being able to cope with distressing circumstances
  • Ability to cope in a pressurised environment and apply practical problem solving skills in everyday situations
  • Competent IT skills and the ability to navigate around various systems and software packages (such as outlook, databases, MS office and the internet)
  • Able to work flexibly on a shift pattern as required and the ability to travel independently between locations to fulfil the requirements of the position
