This post is suitable for registered Mental Health Nurses, Social Workers and Occupational Therapists

Lancaster and Morecambe Community Mental Health Team (CMHT) are looking to recruit a qualified Band 6 Mental Health Nurse. The applicant would need to be registered with a professional regulating body.  The successful candidate may be required to work in either the Non-Psychosis Service Line or the Psychosis Service Line and undertake CMHT tasks as required.

The team serves the population of Lancaster and Morecambe. It is an established team with a friendly and supportive culture. There is a visible and supportive leadership team who are committed to clinical supervision and professional development.

This is a demanding role in a busy service. Ideally, you will have current or previous experience of working within a mental health setting. Clinically confident in working with individuals with complex needs and be able to work effectively in a multi professional context. You should have a collaborative and enthusiastic approach as well as passion and commitment in developing and promoting recovery based approaches for adults with severe and enduring mental health needs.

The post requires that you are a good communicator, and are able to adopt a flexible, adaptable and proactive approach to your work.

Prif ddyletswyddau'r swydd

•    Provides comprehensive assessment, advice and treatment to people with highly complex needs referred to Community Services in accordance with care pathways for given specialism 
•    Delivers specialist interventions in accordance with care pathways for given specialism 
•    Develops packages of individual care and treatment and according to identified need within the Care Co-Ordination and Care Act Framework
•    Works closely with other professionals within and outside the Community Team
•    Actively supports the Team Leader/Advanced Practitioner in the achievement of KPI / CQUIN  and related County Council targets, and maintenance of registration with regulating bodies
•    Takes the lead on specific objectives as directed by the Team Lead and Clinical Team Lead.
•    Provides supervision and direction to practitioners and support staff within community services, undertaking appraisals and supporting the personal development of colleagues
•    Contributes to the screening of referrals and allocation to service lines 
•    Contributes to the assessment of complex cases to support the appropriate allocation of referrals 
•    Supports a culture of ongoing learning and development on an individual, team and service basis to ensure high standards are maintained and, where appropriate, contribute to the delivery of training as appropriate
•    Provide leadership to colleagues acting as a positive role model
