West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

Do you want to work in a high performing team, are you organised and do you enjoy fostering good relationships? If so, we have an exciting opportunity for a Bed Manager to join the Patient Flow Team who will be operationally based across our two inpatient units.

The post holder will be the central point for all referrals for inpatient admissions and will be working within an integrated team alongside discharge coordinator and bed management team to ensure effective patient flow which includes reducing the length of inpatient stay. They will also be responsible for ensuring comprehensive and coordinated discharge planning processes are in place including working with patients with any barriers to their discharge pathway and who are Clinically Ready for Discharge (CRFD). The post holder will also hold a key role in supporting the Acute Trust with efficient transfer once a Decision to Admit has been made to minimise any breaches.

The Patient Flow Team is the single point of access for all referrals for acute inpatient admission. The post holder will co-ordinate and manage patient flow across Adult Acute Inpatient Services (including PICU and Older Adults Services) ensuring the best quality of care for each patient. The post holder will work alongside the Team Manager and Senior Management Team to ensure effective patient flow and a quality lead service.

·                Coordinating all bed requests and enable streamlined capacity management for all boroughs.

·                Provide support to ensure all admissions are purposeful face to face gatekeeping happens with a consistent threshold for admission overseen by the bed management team manager.

Qualifications

  • A relevant Mental Health Professional Qualification (e.g. RMN, Social Worker, OT) with an active registration.

  • Degree in Health Related subject.
  • Management or leadership qualification
  • Mentorship Qualification

Experience

  • Minimum acute mental health experience of 18 months at Deputy/Clinical Team Leader/or practitioner role Band 6.
  • Demonstrable experience of working in front line clinical service and understanding of pressures and ways of working.
  • Experience of working in a community mental health setting

  • Working within a Crisis Team, Liaison Psychiatry or Inpatient Unit at B6.
  • Previous experience of assessing people who present in an acute mental health crisis.

Knowledge

  • In depth knowledge of mental health clinical practice and procedures.
  • Detailed knowledge of national policies and strategies relating to patient flow and the skills to lead and implement these
  • Existing understanding of governance framework, risk assessment, appraisal and mitigation
