Job overview

To provide Specialist Physiotherapy support to the Intermediate care team. 

To provide supervision and leadership to the B6 Physiotherapist, Community Rehab assistants (CRAs) and Community Rehab Practitioners(CRPs) within Intermediate care

To take a lead in the coordination of In service therapy training for the team

To carry and manage a clinical caseload of varying conditions.

To support service manager in service delivery and review

Main duties of the job

The post holder will be working in the Intermediate care Team covering Guildford and Waverley areas. The main base is at Milford Hospital. Work will involve providing assessment and therapy to patients on the Discharge to Assess pathway(D2A) and supporting urgent crisis referrals from G.Ps, UCR/ and H@Hto those with specialist conditions in their own homes.

Working in partnership with members of the multi-disciplinary team, the post holder will manage a caseload of patients independently and be accountable for assessing, planning and implementing Physiotherapy treatment and care to patients referred from the community in crisis, preventing unnecessary hospital admissions, supporting complex discharges from hospitals to enable patients to maximise independence and support patients with ongoing rehabilitation needs. As part of the MDT it will be part of their role to ensure that the patient is at the centre of the care, ensuring patient and carer participation in decision making.

Detailed job description and main responsibilities

Guildford and Waverley-Milford

Are you an Experienced Therapist who is passionate and committed to the delivery of high quality therapy within Intermediate care

We have an exciting opportunity to develop the Physiotherapy service within Intermediate care to ensure patients receive a high standard of rehabilitation in their own homes .The team will assess and treat patients using specialist therapy skills and evidence based practice.

 You will be responsible for the allocation of clinical care for patients on the Intermediate care (ICT) caseload who require specialist physiotherapy and for the supervision and management of the ICT Therapy team .You will be skilled in clinical reasoning, assessment and treatment of patients with a variety of specialist conditions having proven clinical skills to a specialist level.

Working as a team player you will work collaboratively with other leads across Community Services maintaining high professional standards and achieving best outcomes for our patients.

 

The Intermediate Care Team is based at Milford hospital and is a 7-day service. You would be expected to work flexibly in response to service need.

Qualifications

Essential criteria
  • BSc/ Diploma in Physiotherapy or equivalent
  • Registered with Health Professionals Council
  • Evidence of Statutory training(through agency)
Desirable criteria
  • Clinical Educator training-N/A
  • Member of relevant Specialist interest clinical group
  • Member of the CSP

Knowledge & Experience

Essential criteria
  • Experience across the acute and/or community setting, having completed relevant band 6 Physiotherapy competencies/experience
  • Knowledge of a wide range of patient groups including elderly care, orthopaedics, musculoskeletal conditions, neurological conditions, respiratory and falls prevention
  • Experience of working autonomously assessing and diagnosing and treating patients
  • Experience of audit, research and evidence based care
  • Experience of working in a multi- disciplinary team and contributing to team decisions
  • Demonstrate and understanding of National agenda for speciality and have knowledge of the quality standards and NICE recommendations/ NSF/ CQC
Desirable criteria
  • Experience in supervising band 6 Physiotherapist, Community Rehab assistants(CRAs),Community Rehab Practitioners(CRPs)and students
  • Experience of lone working and decision making
