Job overview

As the Patient Coordinator for HCC Surveillance, you will be responsible for supporting the care of patients living with liver cirrhosis through the implementation of a database to identify and track individuals requiring treatment. You will be part of a regionally leading Hepatology service, providing excellent care to patients with liver conditions in the North East.

Main duties of the job

The post-holder will be an excellent team player who is able to work within a multidisciplinary team and able to collate and communicate data to national colleagues in NHS England. The post-holder will have proficient IT skills including Mircrosoft excel and hospital-based information systems. The role will include leading the implementation of a database of patients living with liver cancer. This will include using electronic patient records and data from other sources to identify patients suitable for the database.

Detailed job description and main responsibilities

  • The purpose of this role is to continue the implementation and maintenance of a liver cirrhosis database to enable the hepatology service to continue to deliver exceptional patient care and meet minimal national standards, as outlined in the “NHS England Hepatocellular carcinoma (HCC) surveillance: minimum standards” guidelines.
  • Form strong professional relationships with other clinical teams and partner agencies and work closely with them to enrol all new and existing eligible patients from the hepatology service.
  • Monitoring, and maintaining the database.
  • They will support the patient through the journey from identification and referral to conclusion of their intervention, thus eliminating barriers to care.  
  • The Patient Coordinator will work closely with the wider Hepatology MDT to ensure a patient focussed service and will keep accurate patient and activity records to facilitate reporting back to Commissioners regarding performance and service success.  
  • To evaluate surveillance data, to ensure the hepatology service meets national standards of care and provide regular data submissions to NHS England and the Northern Cancer Alliance.
  • Support the Patient Coordinator (Liver Cancer Surveillance) and to encourage individuals living with, and at risk of, Liver Disease, but who are not yet engaged in treatment, access healthcare services.

Qualifications & Education

Essential criteria
  • • Hold NVQ level 4 or equivalent qualification / experience
  • • Computer literate with a good working knowledge of Microsoft packages
  • • Experience of using spreadsheets to a high level
Desirable criteria
  • • Mental Health First Aid Qualification
  • • IT qualifications – e.g MOST, ECDL

Knowledge & Experience

Essential criteria
  • • An understanding of liver disease and its associated causal factors
  • • An understanding of surveillance programmes and the need for robust patient recall systems to improve standards of care and deliver high quality services
  • • Possess a sound working knowledge of hospital-based information systems, data collection, validation, and reporting
  • • Experience communicating and liaising with patients to support them with throughout clinical pathways
Desirable criteria
  • • Demonstrate a broad understanding of liver disease, including cirrhosis of the liver
  • • Demonstrate knowledge of liver disease identification programmes i.e. aims / outcomes / targets and deadlines

  • • Possess excellent communication skills and be able to work to deadlines
  • • Possess a comprehensive range of practical analytical skills
