Ealing Cognitive Impairment and Dementia Services, within the Older People’s Mental Health Service, based at Acton and Greenford in EALING.

We are looking for dedicated, enthusiastic nurses who are highly motivated and caring professionals who can provide the best possible care with compassion to our patients.
You will have post registration experience, preferable, in the specified care group, and will be someone who can make a significant contribution in supporting the delivery of high quality assessment, investigation and treatment, supporting the team in the day-to-day running of the service whilst being expected to take a lead on a core aspect of service delivery.
The post holder will work with individuals who present with a range of memory related health needs, by providing assessment, information, treatment, review and support through individual and group work as part of a multi-disciplinary team

You will work as a Care coordinator for a group of patients, and be responsible for liaising with other professionals and organisations to ensure that patients’ physical, social and psychological needs are met in an integrated and holistic way.
The successful candidate will have post registration experience within Dementia or community Care, knowledge and experience of older adults with mental health needs or be able to demonstrate your transferrable skills, resilience and commitment through your application and at interview
The post holder will provide specialist assessments, reviews and nursing interventions to service users accessing the Cognitive Impairment & Dementia Service (CIDS). The post holder will ensure the successful implementation and roll out of care for dementia patients

- Professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.
- Evidence of continuous professional development.

Experience

- Experience of working with patients and their carers with complex health and social care difficulties in the designated care group.
- Experience of supervising staff
- Experience of managing clinical caseload will include carrying out initial assessments, treatment efficacy reviews and risk mitigation assessments leading to crisis prevention.
- Experience of providing a range of clinical interventions to people in the designated care group with a variety of health difficulties
- Experience of working consultatively with professionals
- Experience of conducting a range of clinical assessments including risk assessments.
- Experience of partnership working within and across various statutory and non-statutory teams and agencies.
- Previous experience of working with carers and independent / voluntary sectors.
