Community Mental Health Nurse

Band 6

West London NHS Trust provides a full range of mental health, community and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

Qualifications and Training

Essential criteria

Professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.
Evidence of continuous professional development.

Desirable criteria

Dementia and community experience.

Experience

Essential criteria

Experience of working with patients and their carers with complex health and social care difficulties in the designated care group
Experience of supervising staff.
Experience of managing clinical caseload will include carrying out initial assessments, treatment efficacy reviews and risk mitigation assessments leading to crisis prevention.
Experience of providing a range of clinical interventions to people in the designated care group with a variety of health difficulties.
Experience of working consultatively with professionals.
Experience of conducting a range of clinical assessments including risk assessments
Experience of partnership working within and across various statutory and non-statutory teams and agencies.
Previous experience of working with carers and independent / voluntary sectors

Desirable criteria

Experience in working with people with dementia on the ward and in the community.

Knowledge

Essential criteria

A high level of working knowledge of cognitive impairment and dementia.
Ability to work within a Multi-Disciplinary Community Team.
Deputise for the duty manager as appropriate.
Have an excellent understanding of assessment and management of risk.
Effective planning & organisational skills with an ability to prioritise own workload.
Effective communications skills to meet the needs of the post in full.
Effective management and leadership skills.
