Job overview

Exciting opportunities have arisen following a review of provision of mental health services for older people at West London NHS Trust.  We are in the fortunate position of transforming and expanding our service to meet the needs of older people with mental health needs. 

We are looking for a Link Worker (band 4), to join our Older People’s Mental Health (OPMHS) team in Hounslow. If you are interested to join a team of enthusiastic and committed professionals in delivering the NHS Long Term Plan for Older Adults Mental Health services (2019), this will be an ideal role for you.

This post is ideal for a Link Worker who wants to work with older people in the community who have mental health difficulties. We ensure as an employer that the required support you will be part of a new team and as such will be at the forefront of this pioneering work.

Main duties of the job

The Link Worker will be based in Hounslow and they will be undertaking home visits, meetings at the team base and other community sites. 

The Link Worker will work under the supervision higher banded nurse.  The Link Worker will carry a defined caseload of patients. 

They will utilise their clinical knowledge and skills in the assessment and care of Older Adults with functional mental health problems in the community. They will work within a multi-disciplinary team to undertake the facilitation of groups and individual therapeutic activities. 

The Link Worker is expected to contribute to departmental and Trust improvement projects.

Essential criteria
  • Proven evidence of an acceptable level of education/qualification (e.g. CSE, GCSE, O Level) OR A relevant vocational qualification or proven ‘craft’ skills to a minimum NVQ Level 3 or equivalent

Experience
